The Integrated Care Supervisor is responsible for the implementation, management, and daily operations of the integrated care model in the clinic across all service lines. They will advocate for integrated care and work collaboratively with key staff and community members to ensure SCCC Integrated Care Model is successful. The Integrated Care Supervisor is required to perform all duties, and essential functions, of the Integrated Behavioral Health Provider role. In addition, function as the clinical supervisor for Integrated Behavioral Health Providers in the clinic and SBHC, as assigned by DBHW. The Supervisor supports the clinical development of provisionally licensed, and newly hired behavioral health providers.

As a condition of employment, Summit Community Care Clinic requires COVID-19 vaccination of all clinical and non-clinical staff.

Essential Functions:

  • Facilitates partnerships between patients, clinicians, and healthcare teams with a focus on care coordination and integration of treatment internally and externally. Managing grants that support integrated care.
  • Responsible for monitoring and improving internal and community referral processes between all service lines.
  • Responsible for guiding the growth and oversight of the Health Navigation and Internship Program.
  • Monitors programs and adapts care delivery to ensure patient access to high-quality, evidence-based, team-based care across all service lines.
  • Collaborates with department heads to ensure patient needs are being met between departments with program development, integrated care grant deliverables, and supervision of service delivery across all service lines.
  • Develops and monitors essential activities of integrated care, document significant events, and provide analysis of reported data for integrated care services.
  • Oversees the Unite Us Platform. Point of contact for other initiatives regarding community resources.
  • Drives processes and implementation of Social Determinants of Health and its collected data.
  • Consult regularly with SCCC’s Quality department and help strategize on PDSA cycles, and metric performance improvements as applicable.
  • Promotes the philosophy and mission of the Summit Community Care Clinic.
  • Foster clinical supervisees’ professional and clinical development.
  • Ensure the client welfare of those being served by the supervisee, this may include monitoring client care and co-signing notes to ensure that ethical and quality care is being provided.
  • Have obtained all the training for a clinical supervisor or are working towards obtaining clinical supervisor status.
  • Informs and supports annual merit-based reviews.
  • Conducts and supports group supervision for the BH department.
  • Performs other related job duties as assigned by the DBHW.
  • Performs all functions of an integrated BH provider, as outlined in the IBH job description.

Additional Requirements:

  • Grants are a vital part of Care Clinic revenues and directly or indirectly support all positions. This role may have responsibilities that are closely associated with grant deliverables. Grants and grant responsibilities may change over time.
  • Complete all required training as designated by HR and the DBHW.
  • Adhere to all policies within the organization and those outlined within the SCCC Employee Handbook.
  • Comply with all SCCC Vaccine Mandates.
  • Willingness to travel and work at other SCCC locations as determined by the supervisor.
  • Comply with all applicable Credentialing and Privileging requirements timely.
  • Embrace the SCCC Mission and Vision and agree to work within the SCCC Guiding Principles and the principles set by the position supervisor.
  • Must participate in quality assurance/ improvement activities when applicable.
