Opportunity to work for progressive patient and staff focused home care agency in Stratford, CT. Stratford VNA is seeking a RN with home care experience for a Supervisor of Clinical Services/Clinical Manager position. Potential applicant must have a minimum of two years’ experience as an RN in home health, with prior management or supervisory experience preferred. Applicant must have a good understanding of current home health regulations and like working as a role model and team member with the ability to communicate well and motivate others. Solid organizational skills and ability to multi-task are required. Applicant must have a bachelor’s degree in nursing or completed the required community health courses required by the Department of Public Health to be considered.

Education/Experience:
Graduate of accredited school of nursing, with current State of Connecticut RN license.
Bachelor’s degree in nursing preferred or successful completion of community health nursing courses required by the Department of Public Health for Patient Care Manager position.
Minimum two years of home health experience as an RN required. Prior management experience preferred.
Working knowledge of OASIS and home health regulations.
Must be forward thinking and possess good problem solving skills.
Ability to articulate well and present ideas well in a group.
Ability to work independently and within a multidisciplinary team.
Assures cost effective quality care.
Valid driver’s license and proof of valid/current insurance required.
