Job Summary

The Systems Transformation Team in the *Children and Youth with Special Health Needs Section at the Minnesota Department of Health is seeking a passionate, relationship-centered, and driven individual to serve as our Caregiver and Community Impact Coordinator. In this position, you will develop and coordinate statewide long-range plans that ensure that caregivers and communities are equipped to influence systems so children and youth are able to thrive in family structures and communities that support them. The individual in this position will use their knowledge of community engagement, coalition building, and systems and policy change to optimize public heath's role in supporting caregivers. This includes developing and maintaining collaborative relationships with partners, especially those from historically oppressed groups, such as communities of color and indigenous communities, the LGBTQ community, the disability community, immigrants, refugees and others, as you work with our team to gather input and reach consensus in planning and implementing systems change initiatives. Other responsibilities include:

  • Working with partners from the Children and Youth with Special Health Needs (CYSHN) Section, the Child and Family Health (CFH) Division, and other state agency partners to identify priorities and coordinate and leverage resources around supporting caregivers and communities.
  • Developing and executing outreach and engagement plans to ensure caregivers, families, and family-serving organizations from historically oppressed groups, such as communities of color and Indigenous communities, the LGBTQ community, the disability community, immigrants, refugees and others, can access geographically, culturally, and linguistically appropriate home and community-based services and supports.
  • Directing the day-to-day implementation of collaborative and collective efforts focused on bringing together representatives from family serving organizations to influence home and community-based service and support systems of care.
  • Leading and participating on statewide workgroups, councils, and committees to gain insight into needs of caregivers and families of children and youth with special health needs and disabilities and facilitate consensus around plans to improve systems of care.
  • Children and youth with special health needs and disabilities are defined as those who have or are more likely to develop long-term physical, developmental, behavioral, or emotional conditions and who may need extra services and supports compared to other children.

Qualifications

Minimum Qualifications

Only applicants whose resumes clearly demonstrate fulfillment of each of these minimum qualifications will be considered for this position.

  • Four or more years of proven professional experience each of the areas listed below:
  • Planning, implementing and administering public health, social/human services, education, or related programs, or initiatives that serve children and youth with special health needs and disabilities and their families.
  • Considerable knowledge of and experience in project management, strategic planning, coalition building, and program/policy development, implementation, and evaluation.
  • Grant, program or project management experience in public health fields.
  • Coordination and participation on community coalitions, advisory councils or other diverse stakeholder teams.
  • A Bachelor's degree in a closely related field as determined by the agency may substitute one (1) year of experience; master’s degree substitutes for eighteen (18) months of experience; doctorate degree may substitute for two (2) years of experience.

Qualifications to be assessed further during interview:

  • Communication skills sufficient to compose clear, concise and understandable narratives to diverse audiences including families, community members, and professional partners.
  • Ability to develop ideas and recommendations and present them effectively.
  • Ability to organize, plan, and manage time and workload.
  • Knowledge of standard office communication protocols (e.g., email, written and oral communication, developing project summaries).

Preferred Qualifications

  • A Bachelor's or Master's degree in Public Health, Public Administration, Social Work, Human Services, Education or a related field.
  • Knowledge of cultural, racial, geographic, and economic disparities that impact children and youth with special health needs and disibilities and their families.
  • Knowledge of federal, state, and local policies, and initatives programs pertaining to health and well-being of children and youth with special health needs and disabilities and their families.
  • Knowledge of public health policy administration and core governmental public health functions.
  • Demonstrated project leadership and management skills necessary for accomplishing work on teams.
  • Strong interpersonal skills with the ability to maintain effective working relationships with a variety of federal, state, and local and community agencies/organizations and community members.

Additional Requirements

  • This position requires successful completion of a background check and employment reference check.
  • This employment does not require a valid driver’s license, but it does demand travel, which the candidate will be responsible for meeting.
