POSITION SUMMARY
The Mayor’s Initiative Coordinator will support the Mayor’s Initiative on ending Homelessness. The coordinator will develop and coordinate the project. Coordinator will work with the SJCH health team in ensuring patients are screened for homelessness and linked to services addressing Social Determinants of Health. Coordinator will visit sites to meet with patient navigators in the lobby and/or clinic to ensure clients are screened and linked for housing needs, food insecurity, and other legal/social support. The goal is to improve living conditions so that health care outcomes are improved, and we make a positive impact on homelessness.
The position will also be responsible for building relationships with external agencies which includes the Mayor’s Fund Office. The position will collaborate with other agencies and create resources for staff to support clients. The position will be responsible for weekly reports to the Mayor’s Fund and to CPO. Support to staff will include training and supervision. This involves using motivational interviewing among other techniques with a trauma-informed and person-centered care approach. Support to clients may include accessing transportation for clients to appointments, completing housing applications, and conducting eligibility assessments for various services.
The Coordinator will also communicate with staff, recruit, and train staff. The position will develop goals and monitor progress with the support of the CPO. Coordinator will support policies and quality of care to ensure Patient Navigators are providing education, navigation, and follow-up services on housing and supportive services.
