Job Details

Job Location: Tyler, TX
Salary Range: $105,000.00 Salary/year

Description

The Midlevel PA/FNP will provide professional medical services to patients in a clinical setting, working no more than 40 hours per week. They will serve as quality assurance coordinator over other clinic personnel within the agency and complete documentation of all activities. They will assist the Medical Director and other senior staff in developing clinical services and accessing other funding sources, and will follow patients in hospital that do not have an attending physician. Hospital admissions will be limited to no more than 6 at any given time. They will have access to the Medical Director and other participating medical providers to confer, regarding patient treatment and other matters related to medical service.

  • Abides by all SHRT clinical policies to provide quality medical care to all patients in accordance with training and licensure. Performs examination, diagnoses and treats patients 0 to 18 years of age.
  • Completes medical charts after each visit according to the problem-oriented medical records system.
  • Demonstrates patterns of continued learning in order to ensure growth as a practitioner.
  • Attends internal and external meetings, workshops, and conferences, and participates on task forces or committees, as necessary and/or required by SHRT.
  • Performs all duties and services in full compliance with SHRT’s Service Excellence Standards. Adheres to all canons of ethics including SHRT code of conduct for clinical providers.
  • Performs all duties in support of successful EHR/EPM implementation.
  • Monitors clients, following protocol as outlined in the standards of care
  • Provides medication adherence education and appropriate interventions to each client as needed.
  • Provide medical services; work with physician and staff to ensure medical needs are met.
  • Assist with programmatic development and compliance
  • Assist in staff development in the area of Medicine and related topics.
  • Promote team building within the medical staff
  • Assist Medical Directors in oversight of daily clinical operations to ensure staff compliance to Standards of care outcome measures and funding source requirements.
  • Attend regular staff meetings
  • From time to time, provides in-service training.
  • Develop and maintain regional resources for better client care, agency public relations and clinical care strategies.
  • Must adhere to all personnel policies, client rights policies, confidentiality and the agency mission statement and code of ethics.
  • Must attend all required trainings
  • Other duties as assigned.

Qualifications


  • Requires a valid Texas Driver’s License, appropriate driving record and automobile liability insurance that meets the minimum required by the state of Texas.
  • Current Certified Nurse Practitioner license or Physician Assistant from the state of Texas.
  • Prefer Pediatrics Services Experience.
  • Insurability for professional liability.
  • Eligible for Medicaid and Medicare compensation.
  • Dependable transportation.
  • Mental and physical health sufficient to meet the demands of the position.
  • KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:

  • Must be able to adhere to procedures while working in an independent environment.
  • Ability to follow through on all projects a must.
  • Must be a self-starter
  • excellent documentation skills.
