The Position
South Central Behavioral Services (SCBS) currently has an opening for a Care Coordinator in our CCBHC program. A Care Coordinator is someone who provides care coordination to individuals with complex conditions (physical and behavioral health needs) being seen in SCBS’s several programs. The ideal candidate would communicate with persons served, families and providers involved in the care of the person served, including mental health and substance use treatment, case management, primary and psychiatric medical care, housing legal and other agencies. You would participate in treatment team meetings to discuss and address the ongoing needs of the clients and are also responsible for completing daily documentation in our electronic health record. The starting hourly range for this position is $16-$18 per hour, with additional bonuses based on productivity and performance. A hiring bonus of $2500 is available after 6 months of successful employment. Starting wage is contingent upon education and experience. A Bachelor’s Degree in human services is preferred, but we will consider individuals with experience in the field. Valid Nebraska driver’s license is required.
