- Position: Social Worer/LMSW (Non-Profit)
- Location: New York, NY 10039
- Schedule Monday - Friday 9am - 5pm
- Hourly Pay Rate: $28
Requirements:
- Masters Degree in Social Work (MSW)
- NYS LMSW license required.
- 2 years of experience with homeless, chronic health and/or mental health population
Responsibilities:
- Assess clients' substance abuse, health, mental health, and other care needs
- Provide individual and group counseling
- Prepare psycho-socials; Facilitate psycho-education groups in relapse prevention
- Nigating mental health and substance abuse systems, etc. for residents their family and/or support networks
- Provide Comprehensive Case Management services
- Manage discharge planning; engage and support clients 90 days post discharge
Job Type: Full-time
Pay: $28.00 per hour
Expected hours: 40 per week
