Requirements:
- 1-2 year experience working with geriatrics
- Monday-Friday (no weekends) 8:00am-4:30pm
- First time traveler accepted
- Guaranteed hours
- EMR: Epic
- Based on comprehensive assessments, provides perspective on the social, emotional, cognitive and ethnic factors that impact the creation of an individualized integrated plan of care.
- Accesses community and financial resources for participants and ensures continuity of care across the continuum.
- Conducts social work assessment and intervention in other care settings, including the participant's home, as needed.
- Roughly 60% out in the field seeing patients either in their home, AFH, ALF, or SNF’s and 40% onsite in the clinic, but can vary depending on case load.
