  • Hours are 5:00pm-8am, 3 days on, 3 days off

Responsibilities: As an After Hours RN at Snowline Hospice, you will be responsible for providing compassionate and comprehensive nursing care to our patients and their families. Your duties will include: 

  • When waiting for a response from a call to a physician, if no response is received within 15 minutes, repeat request for a call back. If no response has been received in 30 minutes, contact Medical Director and/or the Administrator On Call for additional support.
  • Supports with both hospice and palliative (supportive) care patient visits based on the program for which the patient is receiving care.
  • May perform admission visits after hours based on patient need.
  • Demonstrates appropriate physical and emotional assessment skills for initial and follow-up appraisal of patient status and needs of the family. Possesses an adequate knowledge of clinical disease entities.
  • Demonstrates ability to perform nursing treatments and procedures as prescribed by the patient’s physician.
  • Teaches patient/family care needs and health information regarding pain management, control of nausea vomiting, bowel care, and skin care at their level of understanding.
  • Demonstrates adequate knowledge of specialized diets and medications commonly prescribed to hospice patients.
  • Demonstrates knowledge and awareness of rehabilitative and preventative aspects of care. Ensures patient safety, comfort, and protection.
  • Demonstrates the ability to problem solve and facilitate patient/family decision making.
  • Documents notes of each patient visit reflecting nursing intervention, patient instruction and patient/family response, and specific plan for subsequent visit(s).
  • Documents changes in Physician Orders, communication and coordination with physician and other team members, and submits written Physician Orders in a timely fashion.
  • Documents time, visits, and mileage on time sheet to accurately reflect work performed.
  • Responsible for communicating effectively with other team members.
  • Communicates information regarding the patient to all team members.
  • Carries out job responsibilities and assignments in a manner that meets regulatory compliance and company standards of care.
  • Demonstrates ability to complete job objectives in a timely manner.
  • Demonstrates the ability to work with a variety of belief systems, and honors individual choices and lifestyles.
  • Produces work of sufficient quality.
  • Communicates unresolved work-related problems timely to a member of leadership or human resources and handles confidential information appropriately.
  • Demonstrates positive behavior characteristics such as attitude, initiative, cooperation, adaptability, fairness, ingenuity, and dependability.
  • Attends staff meetings. 
  • Demonstrates open and honest communication for promotion of a positive and supportive work environment.
  • Other duties delegated by the Clinical Director or designee.
