`Job description

The primary function of the Patient Care Coordinator is to serve as an important member of the care team facilitating coordination of care for designated patients, with an emphasis on diagnostic breast health services and health and wellness education to women.

The PC has a role of serving as a trained insurance application assistant to facilitate activities in the area of insurance application. The PC communicates with primary care providers, referral providers, community organizations, the patient/caregiver, and family to assist with the receipt of needed services, ensure communication across the care team and reinforce patient adherence with the care plan. The Patient Care Coordinator works collaboratively in pre-visit preparation, working with care team during and after the patient visit and population health activities. He/she facilitates patient appointments, including those made with labs, diagnostic areas, and specialty physicians.

JOB DUTIES AND RESPONSIBILITIES

Patient Program Coordination

· Pre- Visit Preparation

o Facilitate patient appointments, including those made with labs, diagnostic areas, and specialty physicians where special assistance is needed.

  • As directed by the medical assistant, follow-up with referral provider to obtain reports on labs and diagnostic tests and consultant notes as appropriate. Follow-up with patient/family if test was not done or appointment was not kept and facilitate receipt of test or provider visit.

o Participate in team huddle prior to clinic session to review needed care plans and anticipate flow.

· During Patient Visit

· Review prior care plan goals and progress towards goals with patient/family. Update care plan as appropriate.

· Reinforce care plan with the patient/family in the case of initial visit or initial care plan development.

· Facilitate the completion of depression or other risk screening instruments as appropriate.

· Assist patients to determine their eligibility for appropriate government sponsored programs.

· Facilitate enrollment application process and connect them with enrollment specialist as needed.

· Between Patient Visits

o Notify patients of referral status updates.

o Update referral status to ensure proper closure of loop within EHR system.

o Identify gaps in care for designated patients and facilitate coordination of needed services.

Other Duties

· Manages tracking database to systematically monitor and evaluate outcome of breast health services and reports data to QIQA team in consistent manner.

· Communicates frequently with the care team regarding patient requests, scheduling issues or other aspects of duties.

· Effectively maintains and protects the confidentiality of all medical records and patient information.

· Maintain accuracy of patient demographics and encounter.

· Prepares for next day’s clinic.

· Perform other duties as assigned.

General Agency Duties

· Fosters an environment that promotes multidisciplinary team care consisting of trust and cooperation among all staff.

· Enforces clinic policies and procedures.

· Maintains confidentiality of all patient and employee information to all except designated employees.

· Informs CMO of matters of general interest and problem areas as such are determined or discovered.

EDUCATION & EXPERIENCE REQUIREMENTS

High school graduation or equivalent.

Experience working with underserved and diverse populations.

Fluent in Spanish

PREFERRED EXPERIENCE

Experience working with common health care programs.

Experience working with patient registration procedures.

Basic knowledge of medical or dental terminology.

PHYSICAL DEMANDS:

1. STANDING/WALKING: Up to 10% of the workday consists of stand/walking on tile and cement floors while performing duties.

2. SITTING: Up to 30% of the workday may be spent sitting answering the telephone.

3. LIFTING: Lifting may include up to 5 lbs of items such as charts. 5% of the day involves lifting requiring a full range of motion.

4. CARRYING: Up to 5% of the day may include carrying objects weighing up to 10 lbs.

5. PUSHING/PULLING: Up to 5% of the day may require pushing or pulling equipment, drawers, or carts.

6. CLIMBING: No more than 3% of the time may include climbing one flight of stairs.

7. BALANCING: Up to 1% of the time may be required to use a footstool in retrieving supplies.

8. STOOPING/KNEELING: Up to 5% of the workday is spent stooping or kneeling to open drawers and gathering equipment.

9. BENDING: Up to 25% of the workday may be spent in bending at the waist.

10. CROUCHING/CRAWLING: None required.

11. REACHING/STRETCHING: Up to 3% of the time may require reaching or stretching to retrieve equipment or supplies on shelves.

12. HANDLING: Hand and wrist movement is required 100% of the workday.

13. DEXTERITY: Up to 30% of the workday involves writing or keyboarding, receiving and sending messages.

14. FEELING: Normal tactile feeling is required to perform all job duties.

15. THROWING: none required.

16. TWISTING: Up to 5% of the day may be required in retrieving supplies or equipment.

17. TALKING: Average ability and fluency in English and Spanish is required in communicating with patients, staff, and outside agencies, such as pharmacies and physicians' offices.

18. HEARING: Average ability is required in answering the telephone and performing all job duties.

19. SEEING: Average ability is required to read and follow instructions or use a computer.

COVID-19 CONSIDERATIONS

· All STPCHC employees must be fully vaccinated against COVID-19 as a condition of employment. Successful candidates for this position will be required to submit proof of vaccination against COVID-19 or request an exemption for qualifying medical or religious reasons during the onboarding process. Candidates should not present proof of vaccination until instructed to do so by the hiring department. All STPCHC employees are required to wear masks in the office irrespective of vaccination status.
