Salt River Pima-Maricopa Indian Community is Hiring a Social Worker II (JTR) Near Scottsdale, AZ

Definition: Under general supervision from the JTR RN Manager, the Social Worker II in the JTR division performs professional case management services to assigned adult clients participating in the Office-Based Opioid Treatment (OBOT) program within Behavioral Health Services’ Substance Use Continuum of Care model. Also works closely with clinical program team at JTR and receives guidance from JTR Clinical Manager on clinical outcomes, modalities of care and clinical pathways. Works with adults and their families to facilitate OBOT services by monitoring treatment adherence and progress,coordinating referrals and appointments, and facilitating communication withthe Multidisciplinary Medication-Assisted Treatment (MAT) team. Works with clients by providing navigational tools as resources to help parents develop natural supports and help implement services as needed for each case. This job class is treated as FLSA Exempt

Distinguishing Features: The Social Worker II distinguished from the Social Worker I job class by carrying a higher proportion of difficult or complex cases and providing services in home and in the field.

Essential Functions: Essential functions may vary among position and may include the following tasks, knowledge, skills, abilities, and other characteristics. This list of tasks is ILLUSTRATIVE ONLY and is not intended to be a comprehensive listing of tasks performed by all positions in this classification.

1 .Will be the primary point of contact for all MAT/OBOT referrals, OD Outreach (crisis), Transportation

OBOT Liaison (BHS, D2R providers), ASAM, BAM, GAD/PHQ-9, ROIs (BHS intake packet), Stateside SUD treatment referrals (URC if appropriate), Coordination of Care, (SOP-IOP-PHP-RTC), Treatment Engagement Management (Fidelity to Best Practice, ART Facilitation (or Co-Facilitation with other providers) every 30 days

OBOT Clinic Facilitation, Outpatient MAT Group Facilitation, Organizes and Manages weekly MAT Staffings

2. Provides case management, coordination of care, and support services to individuals and their families. Arranges for provision of and provides referrals to medically necessary behavioral health services. Provides support and assistance to family members in coping with client’s mental health condition. 

3.Confers and coordinates care with contracted providers and representatives from public and private agencies and organizations such as, but not limited to, AHCCCS, Social Security, State/Tribal Welfare offices, legal services, behavioral health hospitals, behavioral health agencies, and residential treatment facilities to assist clients and guardians in applying for and obtaining appropriate needed services. Maintains accurate and current client records through timely documentation of progress notes, treatment plans, and other case file documentation in an electronic health record system. 

4. Maintains statistical data on clients to meet program’s reporting requirements. 

5. Provides transportation for clients as needed. 

6. Participates in clinical staffing and staff meetings. Utilizes supervision and consultation to discuss cases or issues that require additional input and planning. 

7. Participates in training opportunities that will enhance knowledge of tribal case management, behavioral health, child abuse and neglect.

  • Attends professional development meetings/training as directed by immediate supervisor.
  • Keeps professionally current within the field of social work

8. Helps to maintain an environment within the Health and Human Services Department that encourages teamwork, interdependence and ethical behavior.

9. Performs other job-related duties as assigned to maintain and enhance departmental and program operations.

Knowledge, Skills, Abilities and Other Characteristics:

  • Knowledge of the history, culture, laws, rules, customs, and traditions of the Salt River Pima-Maricopa Indian Community.
  • Knowledge of case management techniques and practices as well as crisis intervention methods.
  • Knowledge of IHC policies, practices and procedures.
  • Knowledge of Federal and State laws, rules, regulations and standards that apply to public social service programs.
  • Knowledge of social work principles and concepts.
  • Knowledge of social work methods and techniques, including group and individual counseling, crisis intervention and referrals.
  • Knowledge of Addiction Recovery Principles to include ASAM, Stages of Change, Medication Assisted Treatment
  • Skill interviewing and counseling individuals, families or other groups.
  • Skill with case management including planning, scheduling and time management.
  • Skill recording and documenting case information and case file maintenance.
  • Skill establishing and maintaining effective working relationships with other social work professionals, SRPMIC co-workers, Community officials and clients.
  • Skill evaluating customer needs and utilizing SRPMIC resources or other appropriate referrals to meet client needs.
  • Skill dealing with sensitive, personal issues and maintaining appropriate professional, confidential relationships.
  • Ability to make home visits and provide alternative solutions in order to meet client needs.
  • Ability to communicate effectively both verbally and in writing.
  • Ability to establish and maintain accurate record-keeping systems and procedures including computerized record-keeping.
  • Ability to work under pressure with multiple task deadlines.
  • Ability to use standard office equipment including personal computers, related software such as MS Office and other standard office equipment such as facsimile, photocopier and calculator.
  • Ability to work with a variety of staff members in a team approach to service delivery.

Education and Experience: A Bachelor’s degree in Social Work, Counseling, Psychology, or closely related area AND 3 years full time professional level work experience in the social services field. One (1) year of supervised internship from an accredited college/university can be counted toward the experience requirements.

Equivalency: Any equivalent combination of education and/or experience that would allow the candidate to satisfactorily perform the duties of this position, will be considered.

Underfill Eligibility: An enrolled Community Member whom closely qualifies for the minimum qualifications for a position may be considered for employment under SRPMIC Policy 2-19, Underfill.

  • Due to the confidential and sensitive nature of this position, successful completion of an extensive background investigation is required. Employees in, and applicants applying for, jobs providing direct services to children are subject to the “Community Code of Ordinances”, Chapter 11, “Minors”, Article X. “Investigation of Persons Working With Children”.

  • May be required to work beyond normal work hours including nights, weekends and holidays.
