Definition: Under general supervision from the Behavioral Health Services Manager, performs professional in- home and wrap around case management techniques to assigned adults with general mental health (GMH) and Severe Mental Illness (SMI) concerns within the Adult and Family Services Program. Works with GMH and SMI adults to help improve the quality of their lives (through skill acquisition) and teaching them to utilize available resources and natural supports. This is done through assessment and the development of behaviorally based services plans and providing case management support to the client, family and team members in the implementation of those services. Works GMH and SMI adults by providing navigational tools as resources to help develop natural supports and help implement services as needed for each case. Targets a variety of areas and needs including working with GMH and SMI adults in the field and in home. This job class is treated as FLSA Exempt.

1. Provides case management, coordination of care, and support services to individuals and their families. Arranges for provision of and provides referrals to medically necessary behavioral health services. Provides support and assistance to family members in coping with client’s mental health condition.
2. Maintains statistical data on clients to meet program’s reporting requirements.
3. Provides transportation for clients as needed.
4. Participates in clinical staffing and staff meetings. Utilizes supervision and consultation to discuss cases or issues that require additional input and planning.
5. Participates in training opportunities that will enhance knowledge of tribal case management, behavioral health, child abuse and neglect.
6. Helps to maintain an environment within the Health and Human Services Department that encourages teamwork, interdependence and ethical behavior.
7. Performs other job-related duties as assigned to maintain and enhance departmental and program operations.
