SAINT JOSEPH MEDICAL CENTER is Hiring a Program Supervisor Near Brooklyn, NY

The Program Supervisor is responsible for assisting the Program Director/Manager in the day-to-day supervisory and administrative management of the residential program as well as providing a variety of case management and housing related services to clients to facilitate their adjustment to and integration into the community. He/she is responsible for completing tasks including but not limited to: supervising case management staff to ensure compliance with charting/documentation, preparing monthly statistical reports (e.g. food stamps, food expense, and petty cash), handling client benefits and following up with SSI, Food Stamps, Public Assistance, etc. Program Supervisor is also responsible for conducting/reviewing fire drills, ensuring compliance with safety checklists, as well as ensuring that a clean, safe environment is maintained for all residents. Program Supervisor must monitor and ensure medication compliance, attends and actively participates in monthly and annual trainings/in-services both on and off-sites, and overall represents the department and hospital in a professional manner.

KEY RESPONSIBILITIES;

1. KEY ESSENTIAL FUNCTIONS:

2. Oversee all aspects of the day to day operations of the recovery and support department, establishing effective and efficient operations in accordance with agency, & our funders’ regulations.

3. Residential Services policy and procedures when/if assigned a caseload.

4. Advocates on behalf of clients in resolving issues and mediates problems related to provision of appropriate services including maintaining regular contact with involved agencies and providing in-person outreach when necessary.

5. Assesses clients independent living skills training and/or restorative services training including assisting clients in cooking, cleaning, shopping, menu planning, personal hygiene, budgeting, leisure activities, etc.

6. Monitors client’s benefits and provides budgetary counseling, if necessary (e.g. Medicaid, Social Security, Public Assistance, etc.)

7. Enters maintenance requests as needed and ensures that they are being completed in a timely manner. Follows up with Property Management when/if necessary.

8. Ensures that residence/apartment meets all safety/fire regulations and follows up with vendors as necessary.

9. Conducts/reviews fire drills and self-preservation tests for clients.

10. Assists residents in maintaining a clean, safe, homelike environment.

11. Monitors and ensures medication compliance, including monitoring client’s prescriptions through counting of medication (pills), providing supervision of medication if necessary and providing medication skills training.

12. Manage the recovery support staff, including but not limited to the peer support specialists, mental health aide staff and rehabilitation specialist, through hiring, orientation, training and total performance management ensuring the provision of comprehensive and integrated services.

13. Supervise and coordinate all recovery support staff and workflow; including delegating assignments and monitoring effectiveness to ensure best practices within the program.

14. Manage the staffing requirements for the practice including evaluating staffing needs and participating in the hiring process.

15. Revise and implement efficient tracking of high risk cases, threshold visits, and planned discharges in order to maximize utilization.

16. Participate in clinical and administrative meetings, including but not limited to Aging In Place Tenants, High Risk, Quality Assurance, Incident Review Committee, and staff meetings.

17. Monitor high risk cases and provide clinical recommendations on evidenced-based interventions and practices.

18. Assist in crisis management and oversee subordinate’s incident reporting to appropriate agencies (APS, HRA, HPD, DOHMH…etc.)

19. Review and report of chart compliance with a goal of maintaining 100% compliance.

20. Responsible for the oversight of job performance, guidance of employees and for the direct supervision of designated subordinate staff.

21. Complete daily, weekly and monthly reporting as per funders mandates.

22. Complete special projects and other duties as instructed by the Director

23. Ensure adherence to the scope of service.

24. Lead and oversee efforts to provide services to mitigate risk factors and reduce hospitalizations.

25. Oversee and/or lead program engagement and recovery and wellness events and workshops.

26. Ensure full capacity through external relationship, etc. Completes other assignments given by the Program Director, Program Manager and/or Management team.

27. Adheres to SJMC and departmental policies and procedures; Additionally, adheres to policies related to Infection Control, Patient Safety, Confidentiality, Patient’s Rights, Advance Directives, Disaster Protocol, Safety and Fire Prevention, Attendance and Punctuality

EDUCATIONAL REQUIREMENTS:

· Bachelor Degree in social welfare, behavioral health, or related field required;

· Master’s Degree in social welfare, behavioral health, or related field preferred.

· Licensed professional preferred.

· Assists in collecting monthly apartment checklists from staff. Makes random checks to ensure safety and overall quality conditions in apartments.

· Assist in coordinating community events for clients/tenants including socialization activities

· Cultural competence/cultural humility

· Ensures timely documentation

· Demonstrates proficiency in Microsoft Outlook, Word and Excel

· Demonstrates professionalism

· Demonstrates safe driving skills

· Demonstrates understanding of and compliance with hospital policies and procedures related to Infection Control, Patient Safety, Confidentiality, Patients’ Rights, Advance Directives, Disaster Protocol, Safety and Fire Prevention, Attendance and Punctuality

· Identifies and reduces unsafe practices. This may include reporting unsafe practices, holding accountable the adherence of safe practices, or providing feedback for ideas for safer practices.

· Prevents Hospital Acquired Infections by appropriate hand hygiene. This may include fostering hand hygiene initiatives and holding staff accountable for appropriate hand hygiene.

· Participates in hospital-wide quality initiatives to improve tenant safety.

· Demonstrates knowledge of and participates in, the department’s quality improvement initiatives.

· Demonstrates knowledge of incident reporting procedures and seeks supervisory assistance with same knowledge.

Work Schedule: Full Time Days

· Ability to work flextime some weekends included

REQUIRED EXPERIENCE/SKILLS:

· Minimum two years’ experience in mental health and/or residential setting required.

· Ability to perform in a busy, changing, multi-tasking work environment.

· Excellent written and verbal communication skills.

· Excellent computer skills

· Proficient in data reporting system

· Proficient in Microsoft Outlook, Word and Excel

· Working knowledge of medical and/or behavioral health care environments and terminology

VALID DRIVER'S LICENSE REQUIRED

Pay: $65,000.00 - $68,000.00 per year
