Position Purpose
The Clinical Program Manager provides direct supervision and support to an assigned number of Clinical Supervisors within the RMHS’ Community Transitions Department to ensure the delivery of quality services to the participants in the Momentum, Transition Specialist and Forensic programs. The Clinical Program Manager in conjunction with the Associate Director maintains responsibility for leadership, management, and supervision of both the clinical and non-clinical staff who support the department.
Community Transitions programs assist participants in transitioning from various behavioral health and forensic settings to successful community living. The goal is to ensure that comprehensive, coordinated, easily accessible, culturally informed, trauma informed, and integrated services are available for people in the community; and to minimize the risk of readmissions to hospitals, emergency departments, withdrawal management, Colorado Crisis Services and arrest; and to ensure an individual’s stability in the community.
Essential Duties
Responsible for the coordination, and evaluation of Community Transitions Programs
Directly supervise Clinical Supervisors to support contract provisions including using his/her knowledge of the behavioral health system, clinical interventions and available community resources and peer support services.
Supervisory responsibilities, in accordance with company policies, applicable laws, and the BHA contract to include interviewing, hiring, and training employees; planning, assigning, and directing work.
Assist with the development, monitoring and updating of policies and procedures.
Develop, implement, and manage process for providing initial and recurrent training of staff required by the BHA contract.
Ability to promote the mission, vision, and values of RMHS both in the workplace and in the community.
Responsible for collaborating with Management team to complete monthly, quarterly and annual BHA reporting requirements.
Identify and report to the Associate Director on team activities and issues that may adversely impact the delivery of services under the contract, including submitting critical incident reports to the BHA as required.
Collect, maintain, report, analyze and trend data on performance measures with the help of the Data Analyst to demonstrate effectiveness of Care Management in achieving improved outcomes as identified within the BHA contract.
Establish, update, disseminate, monitor, and ensure compliance with the BHA contract.
Engage stakeholders in regular communication to improve collaboration and continuity of care. Stakeholders can include but are not limited to the following organizations: physical and behavioral health providers, inpatient hospitals, Crisis Service providers such as Emergency Departments and withdrawal management, Criminal Justice organizations, non-profit community organizations, private insurance and Regional Accountable Entities (RAEs) and Community Mental Health Centers (CMHCs).
Ensure the Care Management team is fiscally responsible by having oversight of the departments wrap-around funding and budget needs in conjunction with the Associate Director.

Employees who may drive RMHS or personal vehicles for business purposes. Category B employees may transport other employees but will not transport RMHS clients.
Any Accidents and/or damages while using a personal vehicle for work purposes shall be the sole responsibility of the employee. RMHS is not responsible for any damage incurred to the employee’s personal property.
