SUMMARY As an RN and an interdisciplinary team member, you#ll work to restore health and promote wellness through coordinating comprehensive nursing care for patients of all ages, gender, economic and social status and backgrounds.#This position will ensure that well-coordinated patient centered care occurs, and the patient is able to transition care appropriately. This position cares for patients of all ages from infant to elderly population. QUALIFICATIONS: EDUCATION, LICENSURE, CERTIFICATIONS AND EXPERIENCE Graduate of an accredited school of nursing program with an associate degree or higher. Bachelor#s degree preferred. Current, unrestricted RN Licensure within the State of Minnesota required. One year of experience in clinical utilization required. Experience in case management in a hospital setting preferred. Clinical knowledge of chronic health conditions. Strong written and verbal communication skills. Demonstrated ability to assess and respond to patient and family needs in a timely manner. General knowledge of case management practices, including familiarity with relevant products and services. Basic computer skills, including working knowledge of Microsoft Office Suite and InterQual software. Experience in establishing professional and collaborative relationships with patients, care providers and insurance providers. ESSENTIAL DUTIES AND RESPONSIBILITIES Clinical Utilization Use clinical tools, data and other information to complete assessments of patient needs Create individualized and comprehensive case management and treatment plans Determine insurance eligibility for various treatments and treatment programs Clearly explain the reason for prescribed treatments and treatment plans to patients Educate patients on benefits, services and other resources, including payment options Communicate with patients, physicians and insurance companies to secure successful outcomes Document complex medical information to outline and improve health results Follow case and quality management processes in compliance with regulatory guidelines and company procedures Create an environment of open communication Provide emotional support to patients and families Discharge follow up phone calls to ensure compliance with plan of care Other duties may be assigned as determined by business need Why Choose Riverwood Healthcare Center?
