- Obtain the consumer's medical history and assess for level of care.
- In collaboration with the consumer and physician, formulate a service plan designed to promote good health practices and participation in the recovery process.
- Conduct the review of the service plan with a particular focus on psychiatric and physical health stabilization/management.
- Communicate with physicians to clarify/mediate.
- Perform the nursing process (assessment, identification of problems, planning, implementation, and evaluation).
- Assess signs and symptoms and report pertinent information to the consumer’s FMD or psychiatrist.
- Coordinate Clozaril reporting to adhere to regulations and requirements.
- Perform skilled nursing activities as prescribed by the consumer’s physician and within the purview of the law. Interprets and carries out physician orders accurately and promptly.
- Consult with team leaders and Clinical Coordinator regarding high-risk, problem-prone consumers.
- A graduate of a school of professional Nursing - Associate's degree accepted, BSN preferred.
- A current license to practice in Pennsylvania is required.
- Certification in Psychiatric Nursing specialty from American Nurses Association preferred.
- One year of previous working experience in the nursing field.
- One year of previous work experience in the nursing field and additional experience working within the mental health field are preferred.
