Description

Job Overview: The goal of the reStart Youth Emergency Shelter (YES) is to provide shelter for homeless and runaway youth in an environment that is safe and secure for all participants. The YES Case Manager provides one-on-one case management services to ensure that youth have a safe place to go after they leave YES. The YES Case Manager links youth to educational services, employment services, and other resources in the community. The YES Case Manager assists youth in making and meeting goals and maintains communication with guardians to ensure that youth and family members are working towards shared goals to keep youth safe after their shelter stay. This position serves as part of a weekly on-call rotation to maintain 24/7 staffing in the Youth Emergency Shelter and may need to cover a shift during regular work hours or outside of normal work hours if staff is unavailable.

Responsibilities/Duties:

  • Models positive and healthy relationship building by treating all participants, staff and volunteers equitably with caring, kindness, respect, safety and dignity
  • Works with coworkers, supervisors and staff from other departments to solve problems and to improve processes
  • Oversees youth goal planning and aftercare planning; maintains contact with each youth’s guardian. Coordinates youth discharge plans with the Youth Therapist.
  • Assist youth in accessing community resources including applying for housing programs, food stamps, Medicaid, etc. Track all referrals for monthly statistics.
  • Directly supervise and monitor daily activities of youth; interact with youth while providing a safe structured environment.
  • Documents all significant events/encounters including observed behavior, all telephone contacts, all visits by family, friends, and other agency representatives, medication(s) administered, performance of assigned chores, rule infractions and other required information. Completes the required daily unit reports (staff communication log, staff progress notes, etc.)
  • Administers approved medications according to the guidelines of the Missouri Child Care Association.
  • Interact with youth in an effective, healthy and professional manner to implement daily program activities and to ensure positive peer interactions.
  • Answer telephone and crisis line calls. Complete intake and discharge procedures.
  • Lead regular weekly staff meetings.
  • Lead youth in pre-approved group activities and outings.
  • Conduct periodic room checks.
  • Attend basic training – Orientation, CPR (Cardiac Pulmonary Resuscitation), First Aid, Medication Certification. Attends on-going training sessions to meet 40 hours/year requirements.
  • Complete special projects including cleaning designated areas of the unit.
  • Serve on the on-call rotation and be prepared to come in if a PRN is a no call no show and coverage cannot be found.
  • Other duties as assigned
  • Bending, stooping, walking, standing, sitting and reaching is required. The team member must be able to lift 25 lbs.
  • Be able to travel between locations for meetings and training throughout the local community

Qualifications


Qualifications:

  • High school diploma or GED, and two years of verifiable work experience in a related field; or two years college and verifiable work experience.
  • Must be at least 21 years old.
  • Be able to complete specialized training including First Aid, CPR and Medication Certification.
  • Demonstrated ability to work effectively and collaboratively with a culturally and socioeconomically diverse population of participants, service providers and staff
  • Values diversity and possesses an understanding of multicultural and cultural competencies; understands power, privilege and inequity
  • Knowledge of and the ability to interpret and apply laws, rules, policies and procedures relating to housing assistance programs
  • Adapts to changing business needs, conditions and work responsibilities; responds to change with a positive attitude and a willingness to learn new ways to accomplish work activities and objectives
  • Knowledge and demonstrated ability to provide clinically guided supervision and services related to mental illness, physical disability, substance use, domestic violence and the LGBTQ population.
  • Demonstrates responsible personal and professional conduct; accepts personal responsibility for the quality and timeliness of work
  • Applies effective written and verbal communication techniques to convey clear and timely expectations, listens to people’s ideas and concerns, resolves confrontations, disagreements and complaints in a constructive manner
  •  Ability to exercise independent judgment, using analysis, experience and perspective in solving problems or communicating decisions or actions
  • Effective interpersonal skills, with the ability to work collaboratively to garner trust, respect and confidence from others
  • Ability to work effectively under pressure in a fast paced environment
  • A collaborative team member who is also a strong leader with well-developed organizational and time management skills
  • Ability to develop effective working relationships; works cooperatively, exchanges ideas and addresses issues in a constructive manner
  • MS Office experience, especially Outlook
  • Must pass a criminal records check and pre-employment drug screening
  • Must have valid driver’s license and current automobile insurance

Hours needed:

Business hours, Monday-Friday with weekly on call rotation
