Responsibilities:

• Provide ongoing case management in our Lifeset Program which is a model to address the needs of young people age 17-21 who are transitioning out of care and need individualized skill development and support to transition successfully into adulthood.
• Facilitate Family Support Team meetings.
• Develop individualized care (treatment) plans.
• Coordinate individualized services across all settings.
• Coordinate providers that work in the home, school, and community.
• Document support related activities and services.
• Work a flexible 40 hour/week schedule.

Requirements/Qualifications:

• Bachelor's degree in a human behavioral science and 2 years of experience in a related field.
• Or a Master's degree in a related field.
• Valid driver's license, vehicle insurance, reliable transportation, and a good driving record to allow approval to drive a company vehicle.
• Pass background check and fingerprinting.
