Essential Duties – Duties may vary slightly from program to program, but below is a general listing of duties.

  • Assesses clients’ needs during intake and assessments as well as routine visits; identifies ways to provide resources and to support clients to address identified needs. Maintain a caseload of 25 clients.
  • Works with clients on establishing client-driven goals based on needs assessment.
  • Compiles an accurate and detailed client history.
  • Works with clients from various backgrounds and experiences such as individuals who are unemployed, experiencing homelessness, previously incarcerated individuals, veteran individuals, individuals facing unplanned pregnancies, and/or individuals and families who are facing barriers that prohibit them from meeting their basic needs. 
  • Facilitate in-person and tele-meetings with clients.
  • Provides clients with information and guidance on available resources and support programs; knowledgeable of available community resources and understands how to connect clients with needed services.
  • Conducts follow-up sessions to ensure services are provided. 
  • Collaborates with internal and external partners to identify additional available community services.

Requirements

Basic Qualifications

  • Bachelor degree in social work or a related human service field required (i.e. psychology). Master’s degree preferred.
  • At least 2 years case management experience. 
  • Must possess great communication (verbal and written) and organizational skills; problem solving and conflict resolution skills; knowledge of computers and software programs; ability to multi-task; and the ability to work with diverse populations. 

Skills and Abilities

  • Great communication (verbal and written) and organizational skills.
  • Demonstrated problem solving and conflict resolution skills.
  • Knowledge of computers and software programs (i.e., Outlook, Word, Excel, PowerPoint, SharePoint, etc.) and client management software. Familiarity with Efforts to Outcomes (ETO) client software program is beneficial.
  • Prior experience working with diverse populations. 
  • Possess great time management skills and can handle multiple demands and priorities. 
  • Must have the ability to interact positively with grantee, referral sources, other agencies, and community partners. 
  • Must also have the ability to comprehend, document, and converse in Standard English and demonstrated great sound judgment and reasoning skills. Spanish fluency is a plus.
