Are you a compassionate and skilled nurse looking for an exciting opportunity? Look no further! We are currently hiring for the position of Nurse Care Manager at our reputable home health agency in Palmetto Bay, Florida.
Job Description:
Responsibilities: Nurse Case Manager, you will play a crucial role in coordinating and managing patient care as a part of our clinical team overseeing the services delivered to patients in the community. Working closely with the Director of Nursing services and supervisors of our multi-disciplinary team to develop the right health and rehabilitation plan. Your responsibilities will include:
· Reviewing clinical assessments to evaluate patients’ needs and develop personalized care plans.
· Collaborating with interdisciplinary teams, including nurses, therapists, and physicians, to ensure comprehensive care.
· Reviewing patient records and utilization of services for medical necessity
· Providing education and support to patients and their families.
· Monitoring patient progress and adjusting care plans as needed.
· Documenting patient interactions accurately and maintaining up-to-date records.
· Educating and mentoring the clinical staff and team on the field to ensure up to date with home health care best practices and both state and accreditation agency regulations.
· Advocating for patients’ well-being and ensuring their safety.
· Monitoring and assisting patient and caregiver calls related to the services rendered.
· Review and prepare clinical records for submission to Medicare or other entity for audit requests
Requirements:
RN or LPN Degree: Candidates must hold a valid Registered Nurse (RN) or Licensed Practical Nurse (LPN) degree.
Experience: Preferred 1 year experience in home health care or other related health care field
Computer Skills: Proficiency in using electronic health records (EHR) systems and other relevant software. May be trained as long as basic computer skills knowledge.
Bilingual Skills: Fluent in both English and Spanish.
