PROGRAM: Philadelphia Hepatitis Outreach Project (PHOP)
POSITION OVERVIEW:
The part-time Outreach Specialist will conduct street and venue outreach throughout Philadelphia in targeted communities in partnership with other members of the HCV Street Outreach team. The focus will be on areas where there are high levels of drug trafficking and other high-risk activity to identify substance-involved women and/or men at risk for viral Hepatitis and HIV/AIDS. The Outreach Specialist’s primary responsibility will be to identify and address barriers to sustained HCV treatment adherence through education, screening, behavioral health service, and linkage to treatment for individuals throughout the city of Philadelphia.
