Provident Inc / Mary Ryder Home is Hiring a Behavioral Health Care Coordinator Near St. Louis, MO

Essential Duties & Responsibilities Accountabilities

  • Lead daily programming activities, with the support of the Clinical Supervisor, to enhance the wellbeing and independent living skills of the residents in the program.
  • Document any changes in resident’s behavior, mood, eating habits or medical concerns in the Electronic Health Record System (EHR). Notify the Clinical Supervisor if condition is severe and needing further intervention.
  • Dispense or administer medications, as ordered by a physician, and document in the EHR system.
  • Use strength-based interventions to enhance a resident’s coping skills
  • Teach residents self-care skills, such as proper hygiene and grooming.
  • Help maintain the cleanliness and safety of the facility by active participation and modeling.
  • Teach residents the importance of physical wellness by leading fitness activities.
  • Participate in regular supervision with the Clinical Supervisor.
  • Assist the Clinical Supervisor in resident goal implementation.
  • Participate in quality improvement meetings and program evaluation.
  • Participate in resident treatment planning.
  • Engage residents in one-on-one coaching and skill building activities.
  • Attend doctor’s appointments with residents, when needed for emotional support and to facilitate coordination of care.
  • Admit and escort guests of the Mary Ryder Home.
  • Practice HIPAA and Confidentiality Standards.
  • Attend trainings, as assigned by the Clinical Supervisor.
  • Provide resident oversite and support.

Work Conditions and Environment

  • Work out of the Mary Ryder Home facility, located at 4361 Olive St, Saint Louis, MO 63108
  • Work schedule is a 32-hour work week Fridays, Saturdays, Sundays, and Mondays
  • Ability to travel to other Provident sites, when needed.
  • Must be able to lift up to 50 lbs., including assisting a resident from floor to standing, as needed, with gait belt
  • Have the ability to do repetitive motions by bending, sweeping, standing
  • Position will work daytime hours, typically between the hours of 9:00 am to 6:00 pm

Other Skills Required

  • Experience working with older adults or with individuals who have mental illness is ideal.
  • Knowledge of HIPAA and ability to maintain confidentiality.
  • Strong skills using and navigating computers and software programs. Ability to navigate computer, phone, electronic health record (EHR), Microsoft Office Suite, and other information technology systems independently.
  • Display the ability to remain calm and communicate in a manner to promote well-being of residents and teammates.
  • Participate as a team member, collaborating with and providing excellent customer service to residents, funders, community partners teammates, other Provident employees, and other internal and external customers.
  • Serve as a representative of Provident/Mary Ryder Home, displaying courtesy, tact, consideration and discretion in all interactions with Provident/Mary Ryder Home team members, clients, grantors, funders, auditors, and members of the general public.
  • Ability to foster a positive and inclusive work culture with peers and inclusive treatment environment for residents.
  • Ability to maintain healthy boundaries with residents

Job Requirements and Qualifications

Minimum Education:

  • Bachelor’s degree in social work, counseling, psychology or other health related field

Licenses/Certifications:

  • Current and ongoing Level One Medication Aide Certificate (L1MA). Training & certification provided upon hire.
  • Current and ongoing CPR/AED/First Aid. Training and certification provided upon hire.
  • Applicant must be on the DHSS Family Care Safety Registry to be considered & have a clear background check and drug screening.
  • Physical examination, Hepatitis A shot, and TB testing required prior to first day. Exam, Hep A and TB provided by agency.

Position Summary:

The Behavioral Health Care Coordinator will work with a team of providers to develop and enhance resident’s mental wellbeing and stability, self-sufficiency, and skills to live more independently. Mary Ryder Home is licensed as an Intensive Residential Treatment Setting (IRTS). The goal of Mary Ryder Home is to help support the resident to live more independent lives. The program provides women, ages 55 , who have severe and persistent mental illness with a safe environment to learn daily living skills and receive mental and physical health supports. The Behavioral Health Care Coordinator will be responsible for the development of group and individual activities to support the treatment goals of each resident.

The schedule for this position is full-time (32 hours a week), Friday, Saturdays, Sundays, and Mondays, including holidays that fall during these work days. Holiday pay is provided for eligible work days that fall on major holidays.
