Esse Health is currently seeking a full-time Health Care Coordinator (MA) to assist a team of Nurse Practitioners, Nurses, and Social Workers in delivering care. This position will work out of the Esse Health Central Business Office located in Creve Coeur. The HCC supports the team as they help transition patients throughout the health care journey. This role includes managing referrals, reviewing home health requests, oversight for patients in remote monitoring programs and routine healthcare calls to patients. This position applies the standards of practice and performance in the work setting to achieve positive outcomes and ensures the performed activities are in compliance with all applicable federal and state regulations.

Once training is complete, this position will work 3 days a week out of our Central Business Office located in Creve Coeur and 2 days a week from home.

Qualifications:

• Education: High School Diploma
• Licensure/Certification: Medical Assisting Certification preferred
• Years of Experience: 2 or more years’ experience as a Medical Assistant

Knowledge, Skills & Abilities: 

-Experience talking to patients who have chronic medical conditions

-Knowledge and skills in home health care- skilled nursing and/or therapy services

-Must possess effective written and oral communication skills

-Able to multi-task, complete a high volume of work on a deadline and follow through on assigned tasks

-Attention to detail is essential with excellent time management and organizational skills

-Requires collaboration and cross training with other team members

-Experience with various electronic medical record systems (EMR)

-Proficient with Windows-based PC environment
