Hamilton Madison House is Hiring a BEHAVIORAL HEALTH REFERRAL COORDINATOR Near New York, NY

 

Hamilton-Madison House (HMH) is the leading provider of behavioral health services for New York City’s Asian community and the second largest provider in the country. HMH serves nearly 1,000 community members through a comprehensive program model that understands and incorporates the strengths of individual cultures and addresses the stigma related to mental illness. Services range from psychological services; personalized recovery-oriented services for individuals with mental illness; recovery programming for addiction in six languages and seven dialects; supportive housing for people with mental health issues; and training and internship opportunities for graduate students in social work, mental health counseling, and psychology. Our team of over 30 full-time and 16 part-time staff speak Cantonese, Mandarin, Fujianese, Korean, Japanese, Vietnamese, Toishanese, Taiwanese, Spanish, Khmer, and Cambodian. 

The Behavioral Health Referral Coordinator will serve as the initial point of contact for patients seeking behavioral health services throughout the pre-admission and admission process to all behavioral health programs. They will coordinate all aspects of the initial consultation and intake assessment process, and collaborate with the Administrative Manager and Clinical Directors to triage and connect patients with services. They will conduct screenings and pre-admission consultation with new patients, families and referral sources, as well as provide one-on-one navigation to existing patients and coordinate intake appointments with program directors.

Responsibilities:

  • Coordinate all referrals for mental health services, including direct patient referrals, hospital referrals, internal referrals, and referrals from outside agencies.
  • Complete pre-admission paperwork for all patients seeking behavioral health services.
  • Utilize evidenced-based practices and interventions to screen all new clients and provide referrals as appropriate to internal and external services.
  • Record and maintain referral log spreadsheet and monitor/report on data/outcomes.
  • Streamline the intake & discharge process while working closely with the management team to report trends and quality measures.
  • Partner with the Administrative Manager and Clinical Director to assign cases to therapists as indicated based on assessing clinical compatibility and patient needs.
  • Collaborate with administrative staff to ensure initial appointments with providers are expedited and to promote engagement in the treatment process.
  • Request/obtain medical/patient records from previous and/or referring facilities.
  • Attend professional meetings, educational conferences, and workshops, to maintain and improve professional competence.
  • Immediately report any incidents, issues, or unusual occurrences to the Clinical Director.
  • Maintain collaborative and compliant documentation that meets OMH and HMH expectations. Maintain accurate and complete records as required.
  • Actively participate on an interdisciplinary team.
  • Other special projects/assignments as needed.

Minimum Qualifications: 

  • A baccalaureate degree from an accredited college; or at least 3 years of experience working in human services with individuals with behavioral health and/or substance use disorders. 
  • Preferred: Master’s degree in Social Work, Counseling, or Psychology with a license in New York (LCSW, LMHC, LPC)
  • Demonstrated cultural proficiency and experience working with communities of color and/or communities in urban, underserved neighborhoods that bear a disproportionately high burden of illness. 
  • Positive attitude and strong interpersonal, writing and communication skills. 
  • Understanding of racial/social justice and equity issues. 
  • Strong time management skills with attention to detail and ability to prioritize. 
  • Able to meet the demands of a busy clinic practice in both serving patients and documenting the work done in a timely manner.
  • Proficiency in Microsoft Office Suite (Word, Excel, and PowerPoint) and electronic medical records, and willingness to learn new systems as needed. 
  • Working experience with electronic medical records software.
  • Bilingual speaking abilities Cantonese or Mandarin preferred, but not required. 

Compensation: $24.60/hour to $28.20/hour (depending on education and experience)

Hours: Monday to Friday, 30 hours/week 

Excellent benefits including medical, dental, and vision insurance, 15 vacation days, 12 sick days, 3 personal days, and 12 paid agency-wide holidays. Depending on the role, HMH offers a flexible work schedule
