POSITION SUMMARY

The Community Registered Nursepartners with Health Services leadership to oversee the clinical aspects of thehealth services department for assigned community and ensures that residentcare needs are met by completing assessments, coordinating resident care,managing health services systems, and delegating nursing tasks. Promotes a positive physical and psychosocial environment for the residents.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  1. Creates a resident-centered approach to the planning and delivery of services to residents, supporting residents’ rights toparticipate in decision-making and treating residents with dignity and respect.
  2. Performsassessments on health, functional and psychosocial status of residents.Participates in the development ofindividualized service plans. Proactively manages care and services for eachresident, maximizing the resident’s opportunity to remain in the assisted living environment. Problem-solves as partof the management team to case manage and meet the needs of residents.
  3. Assesses or evaluates residents with reported changes of condition. Provides direction, service planning,and coordination of care to meet the residents’ changing needs.
  4. Collaborateswith the management team on the move in of prospective residents, taking intoconsideration the needs of the prospective resident, current community acuity, and staffing levels. Provides in-person pre-move inassessments, if necessary.
  5. Ensuresthat resident care needs are met by coordinating services with otherprofessionals, including physicians, discharge planners, nurses, home healthagency staff, case managers, hospice, and state agency personnel. Oversees the arrangement of ancillary services such as pharmacy, podiatry, lab, andx-ray as needed.
  6. Delegatesnursing tasks as allowed under applicable state nursing laws and practice acts,communicating regularly with residents, their families, and their health care practitioners.
  7. Oversees the documentation and communication of resident care and services. Assists the leadership team to ensure resident recordsare completed in a timely manner and maintained according to company policiesand state regulations.
  8. Maintains and oversees medication services including staff training, staff oversight, pharmacycoordination, medication room, and medication systems.
  9. Partners in the investigation and root cause analysisof incidents. Collaborates with the management team on the development and implementation of appropriate interventions.
  10. Assists in the management of Infection Control. Assists with providing training to staff on infection controlpractices, investigates, and reports incidents of infection and tracksinfection data.
  11. Provides on-call support and phone triage for team members’ questions regarding resident concerns.
  12. Participates in leadership team by actively contributing in decision making, incident follow up, building and company-wide initiatives and attending leadership team meetings such as daily stand up, Systems Monitoring and Resident Tracking (SMART), Quality Assurance Performance Improvement (QAPI), and othermeetings as required.
  13. Acts as an ambassador of Prestige by representing communities in a positive manner to outside agencies and participating incommunity events and social functions.
  14. Maintains up-to-date knowledge of all rules,regulations and policies pertaining to resident care. Responsible to stayinformed of industry and health care trends and standards, applying newknowledge to meet the changing needs of residents. Seeks out additional expertise and advice when needed to assure residents’ needs are met and proper practicesfollowed.
  15. Performs other duties as assigned.

QUALIFICATIONS AND EXPERIENCE

Must have current, unencumbered RN license in the state of practice. Delegation experience in thestate assigned preferred. Experience in geriatric nursing, medicationadministration, case management, and training and staff development preferred.

EDUCATION

Registered Nurse license with a Bachelor of Science in Nursing or Associate Degree in Nursing, may also be a diploma RN.
