The Outreach Specialist is a community-based position created with the intent of "meeting people where they are at", both physically and psychologically, with dignity compassion, and unconditional positive regard. This individual must be comfortable transporting individuals in their personal vehicle, and should use trauma-informed approaches while refraining from presuming to know what an individual needs/wants or judging their choices or decision-making, by providing client-centered support.

Job duties include but are not limited to:

  • Be visible, be present, and being persistent
  • Use trauma-informed language and approaches
  • Be a fierce advocate for and with individuals
  • Utilize harm-reduction practices, motivation interviewing, active listening, de-escalation, and crisis interventions
  • Network with agencies and community partners that provide essential resources including shelters, hospitals, treatment providers, and probation and parole offices.
  • Be a good steward of knowledge about resources to other outreach works and community partners.
  • Identify clients’ strengths, treatment needs, and any supports (natural or otherwise) that they have
  • Determine clients’ needs by an assessment of areas of daily living and functioning to include: mental health, physical health, substance abuse, employment, housing, and skills in areas such as socialization, problem solving, coping, money management, and time management.
  • Assist clients’ in reinvigorating a healthy system of emotional and social supports; provide a systemic intervention of helping families and others who may once have provided significant support
  • Create collaborative treatment plan with clients in an effort to identify goals they wish to achieve while working with PFP
  • Inform clients of expectations regarding services, focus on services and supports PFP can offer in achieving goals
  • Coordinate correspondence with other agencies to obtain and provide client clinical records as needed
  • Collect and analyze quality assurance data from clinical records and make recommendations for improvement of care; communicates quality assurance findings to clinical staff to maintain completeness and timeliness of client records
  • Maintains quality assurance records and files according to agency certification and accreditation requirements
  • Attend weekly meetings Outreach/ Legal Outreach Team meetings
  • Attend weekly supervision with direct supervisor
  • Complete required trainings within established timeframes
  • Complete annual training hour requirements as outlined in the Employee Handbook
  • Performs other related tasks as assigned by the supervisor
