Case Manager- Day Habilitation

Pay $21.25 / HOUR

Schedule: Monday-Friday 7:30am-3:30pm NO WEEKENDS, NO HOLIDAY, NO NIGHTS!!

Location: 1040 Eastern Avenue, Fall River

People, Incorporated is looking for an individual to join our team as Case Manager for our Day Habilitation Program. Case Manager assumes primary responsibility for each member on an assigned caseload and coordinates the implementation of the program Service Plan and maintains each member’s records.

Essential Functions

• Develop and ensure the implementation of the Program Service Plan.
• Serve as liaison between the program and each individual’s support system.
• Complete and submit quarterly progress documentation and maintain and update all service records.
• Attend meeting, training and continuing education or other appropriate educational programs as required and requested.
• Attend interdisciplinary team meetings as needed to participate in curriculum design.

Qualifications

• Knowledge obtained through attainment high school diploma or equivalent
• At least one-year case management experience in a program for developmentally disabled adults, or three years’ experience in Human Services or a related field.
• Skill in both written and oral communication.
• Ability to working both independently and as a team member.
• Ability to effectively participate in the design, implementation, and coordination of services.
• Ability to complete and disseminate all required forms and documents.
• A qualifying criminal background check, driving record, and a valid driver’s license required.
