PermanantJob Description : JOB DESCRIPTIONMaintaining efficient and correct medical records within the electronic medical records system.

Works as part of a care team that practices evidenced based medicine in a Patient Centered Medical Home model of care.QUALIFICATIONSEducation : High School Diploma.

Medical office experience preferred. Experience with medical office systems, medical terminology, customer service and medical equipment preferred.

Other : Knowledge of coordinating care with other agencies, computer experience, must be able to demonstrate excellent communications skills;

both written and verbal. Must have diplomacy, tact, and a positive approach to problem solving. Organizational skills, professionalism, along with confidentiality a must.

Preferred Skills : DUTIESMaintains fax in / out box for all PATHS offices, and assigns appropriate documents to nurses, providers, referral coordinator, and Navigators.

Enters all appropriate data into the Electronic Medical Records system.Maintains medical record releases for patients with the intent to obtain outside records from other facilities.
