The Clinical RN Manager, coordinates all aspects of patient and family care. Serves as team leader, providing direction and supervision for the interdisciplinary team members to promote OpusCare values in proactive patient care management activities. Qualifications Registered Nurse license in state(s) practicing. Current Cardio Pulmonary Resuscitation (CPR) certification. Minimum two years’ experience in hospice, home health, general medical surgical, oncology or patient population specific (i.e.: geriatrics) nursing. Minimum one year of supervisory experience. Must have reliable automobile, valid driver's license, and the minimum state required liability automobile insurance. Demonstrated commitment to OpusCare philosophy of care, values, mission statement and organizational culture. Ability to travel throughout the State of Texas.
--Manage the Interdisciplinary Group, ensuring appropriate delivery of Hospice services to patient/family units.
--Coordinate and supervise field staff to provide optimum quality patient/family care.
--Ensure appropriate development and maintenance of a written plan of care for each patient/family unit and updates twice monthly or more often if needed.
--Make periodic visits to patient/family units for the purpose of evaluating nursing care, the interdisciplinary group and general quality of service provided and perform annual supervisory visits with all field nurses on team and Hospice Aides.
--Assess to ensure that appropriate and adequate psychological, emotional and spiritual support is provided to the patient/family unit.
--Implement staffing patterns taking into consideration budget factors, and patient acuity.
--Responsible for implementation of Hospice Policies and Procedures and for awareness of Federal and State and Hospice rules and regulations and CHAP standards.
--Maintain assignment map up to date based on census growth.
--Attend IDG meetings on a weekly basis.
--Have monthly staff meetings with teams assigned.
--Provide training as needed to staff.
