AIP Hospice is Hiring a RN Case Manager Hospice Near San Antonio, TX
Competitive Compensation: Will be commensurate with experience.
RNCM - Monday to Friday 0900-1700, with On-Call Weekend Rotation every 4th week. Week nights covered for on call. Hospice experience preferred . Spanish speaking a plus. This is a full-time salaried position with PRN fees for after hours or On-Call visits, etc. Generous mileage reimbursement at .0625 cents per mile. Position pays for $75K-$85K with lower than average patient caseload. Agency uses Consolo EMR.
Job Summary: Primary functions are to administer nursing care for patients of all ages in their place of residence, coordinate care with the interdisciplinary group, patient, family and referring agency; and assume the responsibility for coordination of care.
Job Qualifications: Education: Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing Licensure: Current license as a registered nurse (RN) and/or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC), current Drivers License.
Experience: Two years experience as a Registered Nurse in a clinical care setting preferred, hospice preferred.
Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Specialized skills preferred in palliation and end-of-life. Proof of current CPR. Must read, write and comprehend English.
Transportation: Reliable transportation. Valid and current auto liability insurance.
Environmental and Working Conditions: Works in patients home in various conditions; possible exposure to blood and bodily fluids and infections diseases; ability to work flexible schedule; ability to travel locally; some exposure to unpleasant weather; PRN emergency call.
Physical and Mental Effort: Prolonged standing and walking required, with ability to lift up to 50 lbs and move patients. Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition; meet patient/family individualized psycho social needs. Requires hand-eye coordination and manual dexterity.
