Most hospice care is provided at home, where people most want to be when they are ill. In addition, hospice care can also be provided in a nursing or residential facility and in certain circumstances, in a hospital or hospice management residence

Position Overview:

At the highest level of OpusCare Employees must be committed to hospice philosophy and mission and provide outstanding customer service to both internal and external customers. As a member of the Interdisciplinary team, the “Admissions Nurse” works under the direction of the Admissions Director and/or in her/his absence the Director of Clinical Management, and the patient’s/Hospice physician in the provision of skilled nursing care & services, assessment, planning and care to maximize the comfort and health of patients and families. Responsible for providing nursing care and services, assessing and identifying patient/family needs, and providing skilled care and supportive services in accordance with the patient’s physician’s orders and plan of care and Opuscare’s policies and procedures.

Essential Duties:

· Demonstrate nursing practice skills and the ability to assess and respond to the needs of patients and families in varied settings and the ability to execute a supportive, sensitive approach to patient/family/personal caregiver (PCG) or significant other (S/O) needs.

· Have excellent communicating skills, both orally and in writing, to include the ability to liaison all aspects of patient care/services, obtain cooperation from co-workers and other services sources, as directed or so required.

· Have the ability to work diligently and become knowledgeable and adhere to program policies and related procedures and of pertinent State and Federal Hospice regulations.

· Have the ability to cope with patient/family/personal caregiver (PCG) or significant other (S/O) emotional stress and be tolerant of individual lifestyles.

· Other duties as assigned by Management based on company needs.

· Available to work weekends based on company needs.

· Availability to work across Miami Dade County based on company needs.

· Shares weekend rotation, as assigned/requested to ensure timely response to new admissions and/or patient/family requests.

Responsibilities:

· Assume nursing responsibility for the assessment, planning of the patient's admission.

· Initiate, and relate to the office staff, communication with attending physicians, other Hospice staff members, and other agencies, as needed, to coordinate optimal care and use of resources for the patient/family.

· Obtain data on physical, psychological, social, and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care.

· Maintain up to date patient records so that problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur.

· Seek input from other team members regarding the patient plan of care to obtain additional knowledge and support.

· Accept responsibility for coordinating physical care of the patient by teaching primary caregivers, volunteers.

· Attend departmental meetings as requested.

· Complete nursing assessment within 24 hours.

· Provides information to department director(s) of unusual or potentially problematic patient/family issues, assisting during the problem resolution phase and providing appropriate follow-up intervention.

· Participates in Opuscare’s orientation and training programs for professional staff.

· Demonstrates knowledge of Hospice rules and regulations and of program policies and procedures.

Minimum Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. · Graduate of an accredited school of nursing; minimum of ADN, Bachelor’s Degree in Nursing preferred.

· Current State of Florida RN license.

· Two years recent (within the past five years) nursing experience in acute, long-term care or home care/hospice, with at least one in hospice or homecare preferred.

· Must be able to work independently, have demonstrated patient care planning and patient teaching skills and strong written and verbal communication skills.

· CEU’s – HIV, OSHA, Domestic Violence, Medical Errors, Alzheimer’s (4 hours), Laws & Rules, Impairment in the Workplace, Human Trafficking

· Current CPR certification.

· Excellent oral and written communications skills in English. Bilingual (English / Spanish).

· Must have general computer skills and a working knowledge of MS Office, Outlook, and the internet.

· Belief in the hospice concept.

· Must be 18 years or older.

· Pass the drug test and background check.

· Ability and willingness to work a flexible full-time or part-time schedule that frequently includes weekends and holidays and after hours.

· Must be able to provide own transportation to various locations in service areas as required by duties.

· Must be able to use facsimile, write and transcribe information accurately

· Valid FL Drivers License and daily access to a reliable and insured vehicle.

Pay: $40.00 per hour
