We aspire to aid the growth of our company by welcoming the most qualified and deserving candidates aboard. Optima is seeking a medical assistant for our Care Management department. This individual will be expected to monitor a specific group of patient’s care and support them in achieving short/long term health goals. The Care Manager will coordinate with providers to ensure patients’ needs are met. A working knowledge of the clinical field is desired for this position.
Care Manager Responsibilities:
- Manage a roster of patients who are enrolled in the Chronic Care Management (CCM) and Remote Patient Monitoring program (RPM).
- Act as a direct liaison between the patient and the office/provider.
- Scheduling and confirming appointments in office for the patient.
- Have a full understanding of each patient’s individual care plan and making sure they are following it.
- Record patient data daily and alert healthcare providers of abnormal reports.
Care Manager Qualifications:
- At least 1 year of clinical work experience as a Medical Assistant
- At least 1 year of EMR experience; eClinicalWorks preferred
- High level of professionalism
- Excellent written and verbal communication skills
- Strong customer service skills
- Medical Assistant Course Completion Diploma or CMA designation required
