Position Summary:

Provide intensive case management and crisis intervention services to homeless young adults in the Transitional Aged Youth Emergency Shelter. Assist youth in locating permanent housing, developing skills and transition plans.

Job Duties & Responsibilities:

Program Orientation

• Facilitate intake process (complete paperwork, acquire necessary signatures and release of information, review program handbook with participants).

Direct Youth Services

• Direct oversight of youth: Oversight of chores, meals, hygiene & daily tasks.
• Dispense medication to clients.
• Provide advocacy, information referral, crisis intervention and other direct services.
• Provide case management, including life skills assessments, and develop Transition Plans.
• Assist participant with setting personal, program, educational and employment goals.
• Facilitate life skills groups for participants.
• Coordinate each participant’s implementation of health and medical maintenance and prevention.
• Coordinate educational guidance and placement service, ensuring each participant finishes high school or receives a GED prior to graduation from the program.
• Teach or coordinate workshops including budgeting food purchase and preparation, time management, health, leadership, socialization skills, building healthy relationship, etc.
• Transport participants to necessary appointments.

Resource and Support

• Maintain linkages with community referral sources.
• Link participant with community and professional contacts (transportation services, legal services, etc.)
• Coordinate mental health and referrals for each participant as needed.
• Assist in provision of special independent living skills events, including community building and celebrations.

Team Coordination

• Meet with the Director and Main STAY staff weekly to discuss all aspects of program and participants.
• Responsible for giving violation notices and recommending to the Director to dismiss participants from the program.
• Maintain all statistical information, with complete and up-to-date paperwork and updating of case files.
• Attend all staff meetings, including training and supervision.
• Ensure standards of quality.

Exit Process and Aftercare

• Facilitate exit plan and complete exit data.
• Provide follow-up/aftercare services to exiting participants.
• Other duties as assigned.

Skills and Qualifications:

1. Bachelor’s Degree in social work or related field and/or three years’ experience working with youth.

2. Have knowledge of community resources.

3. Good written and organizational skills.

4. Able to work with at-risk population.

5. Able to clear state criminal background check.

6. Valid CA driver’s license and clean driving record.

Work environment:

· Typical indoor office environment

· Moderate noise level

Physical Demands:

· Must be able to remain in a stationary position

· Light work that includes moving objects up to 20lbs

· The person is this position needs to occasionally move about inside the office to access file cabinets, office machinery, etc.
