Responsibilities:
- Provide case management services to a caseload of clients
- Coordinate and monitor client services and progress
- Collaborate with healthcare professionals to ensure comprehensive care
- Administer medications and monitor medication adherence
- Provide catheter care and assist with other medical procedures as needed
- Monitor vital signs and assess overall client health
- Maintain accurate and up-to-date client records
- Advocate for clients' needs and rights
- Assist with patient scheduling and ensure effective communication between physicians and other healthcare professionals.

-Assist with intake referral processes and coordination of patient care.

Requirements:
- Previous experience in case management or a related field
- Knowledge of critical care systems and procedures
- Ability to catheterize patients and provide catheter/wound care
- Proficiency in vital signs monitoring
- Experience with an EMR charting system
- Strong knowledge of human physiology and medical terminology
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team

Pay: $30.00 - $35.00 per hour
