LVN Case Manager:

Liaison for Helping Hearts Hulen, IEHP and other community agencies. Assist in coordination of care with IDT. Attend monthly Treatment Team and Emergency MDT meetings. Provide counseling, linkage and consultation to health care providers, benefit programs and community resources. Conduct daily visits with members to assess needs and discuss treatment goals. Identify discharge barriers and assist members in meeting goals. Receives documentation from providers and follows up with patients regarding their treatment plan. Informs all parties involved on the processes that are being used to treat the member. Inform and educate the member and family members involved in ongoing treatment plans for the members and provide the patient with written information regarding their treatment plan and other available resources. Maintain complete and accurate records on members. Maintain member confidentiality in treatment plans and reporting as appropriate. Understands and abides by HIPAA guidelines. Answer members questions about their care and treatment plans. Address any member concerns with appropriate in-house staff. Promote a safe and secure environment to meet the needs of the member. Participate in discharge planning and assist in successful transition to alternate levels of care. This position reports to the Administrator.

Modeling appropriate social behavior and positive use of free time with recreational activity both in the home and the community, and identification of community resources. Ensure that the members comply with house rules, treatment plans which include personalized goals, appropriate interpersonal relationships, and fire and safety protocols. Facilitate communication with members including how to follow the program guidelines and protocols, implement day-to-day structure which may include redirection, and verbal intervention where applicable. Submit significant and routine paperwork such as progress notes, incident reports, medication logs, Staff change reports, maintenance requests, etc. as required by the program. Attention to detail is important for these clinical records. Be present and awake at a Helping Hearts Hulen facility when a member is on the premises. Complete monthly in-service training. This position reports to the Administrator. Provide support and transportation to the members as they integrate into the community. This includes community functions such as shopping, groups and activities in the community. Educate members related to independent living skills and activities of daily living, goal setting, budgeting, vocational skills, wellness, and social and recreational activities, etc. either individually or in a group setting. Provide support to the Program Administrator as needed to maximize coordination of care and creating effective treatment plans. Provide coordination of care with IEHP, Behavioral Health and medical providers as needed by the Administrator, which can include transportation to physician appointments, Urgent Care, Emergency Room, etc. Carry out activities and outings for the members including opportunities for moderate exercise such as walks, hikes, swimming in public pools, etc. This position reports to the Administrator.

Requirements:

• Possession of First Aid Certification
• Must be at least 21 years of age and have a verified GED or High School Diploma (copy maintained in employee file).
• Must have a LVN License, Social Work or Case Management experience.
• One-year full time work experience in a community program that serves members who have a mental illness, with the experience in the direct provision of services to members and at least four months in a position of supervising direct care staff.
• Completed and been cleared through Concentra Pre-Employment physical and have a clean TB skin test or Chest X-Ray (if previous skin reaction), and negative five panel drug screen test.
• Clear Criminal Record Clearance or approved exception (through Livescan) that is verified through Helping Hearts and assigned to Helping Hearts Hulen facility.
• Has a clean driving record verified through Helping Hearts Insurance Agent and maintains standard of care for vehicle operation and driver training for 15 passenger van.
• Duties and requirements must be consistent with Title 22 SECTION 81064.1 and Title 9 SECTIONS 532.6 (f), (g), and (i).
