Purpose: To compassionately provide individuals with diversion alternatives to prosecution through supportive services that address needs and increase stability to reduce harm.

Job Summary: The RFS Diversion Case Manager (RDCM) specializes in case management services that support individuals that enter the Commonwealth?s Diversion Program. The RDCM will provide risk-based case management services to clients and make appropriate referrals for other client needs. The RDCM is responsible for documenting all client services offered. The RDCM will organize and facilitate classes, groups, and special programs as required. Service area: Fairfax County

Key Responsibilities

• Writing policies and guidelines to execute the Diversion program
• Developing, evaluating, and modifying existing policies
• Perform RNR assessment that includes OST criminogenic risk assessments and a needs assessment with clients
• Provide risk-based case management services, including assisting clients with obtaining identification, employment applications, resume, and cover letter development, interview preparation, job search and retention skills, and budgeting and financial planning in the Community
• Assist in the training and orientation of staff, volunteers, and interns as requested
• Provide case management, intakes, assessments, record checks, site placement, monitoring, and follow-up of clients to ensure successful program completion
• Appear in court to screen potential clients, report client progress, and assist the court with cases referred to OAR
• Ability to build and maintain community partnerships
• Attend organizational and community-based meetings as required to ensure appropriate service delivery
• Develop, attend, and participate in forums about the diversion program

• Facilitate classes for clients
• Provide direct assistance to clients for employment-related needs and facilitate the Work Empowerment Program (WEP)
• Follow up with clients on their employment status
• Communicate the case management services available
• Create and manage client files according to agency policy and procedure
• Document all services provided
• Obtain, track, and disseminate resources, and incentives such as transportation assistance, gift cards, and personal care items
• Develop, attend, and participate in forums about the diversion program
• Identify best practices to ensure effective service delivery
