Northwest Medical Homes, LLC is Hiring a Triage Nurse(RN/LPN) Near Springfield, OR

5-Star Northwest Medical Homes, LLC (formerly known as Springfield Family Physicians) is looking for an energetic, hardworking, and experienced LPN or RN to join our exceptional medical support staff. This person must have excellent telephone skills, medical and medication knowledge, and have experience with an Electronic Health Record. Duties in this position include triaging phone calls from patients, conducting patient assessments and refilling medications following predetermined protocols. Top candidates use clinical knowledge, experience, and best practices to provide medical advice, while following established procedures and protocols for escalation and documentation. Hours: Monday- Friday.

Primary responsibilities/everyday tasks:

  • Triaging phone calls and providing medical advice following predetermined protocols
  • Answering pharmacy calls for clarifications, concerns or questions
  • Answering calls concerning lab results or test results and review those with the patient
  • Taking calls from labs with urgent results, tests not done, concerns, or diagnosis codes needed
  • Relaying returned call messages and answering any questions that the patient may have
  • Taking requests from patients concerning medication changes or requests
  • Taking high priority calls from patients with serious medical conditions, or rapidly deteriorating conditions requiring immediate need, and triage those calls to determine if emergency room or immediate visit is needed i.e. stroke or cardiac arrest
  • Doing "today's information"
  • Assisting providers with any problems when they are off or working from home, and may need assistance with a patient
