IDDS Resource Coordinator

NKHS is a Designated Agency contracted with the State of Vermont to provide the highest quality of service in the areas of adults with intellectual/developmental disabilities, children and youth with serious emotional disabilities, and adults with mental health and substance use challenges. We serve the populations of Caledonia, Essex, and Orleans counties through a wide variety of targeted programs for individuals of all ages. NKHS has over 500 employees providing case management, community and home supports, residential care, psychiatry, medication management, therapy, vocational supports, school based counseling, emergency care, and respite services. RESPONSIBILITIES & SCOPE
1. Housing Support Services: Provide tailored housing support services to meet the unique needs of program participants.
2. Program Knowledge: Demonstrate understanding of IDDS and AFC program requirements, including waiver funding, or willingness to acquire necessary information.
3. Collaboration: Effectively collaborate with agency staff and community resources to address unique needs of program participants.
4. Record Keeping: Maintain accurate and up-to-date medical records in compliance with State and Federal guidelines, utilizing Electronic Medical Records (EMR) systems.
5. Teamwork: Demonstrate strong teamwork skills, working collaboratively with program staff, families, guardians, and team members to identify and support housing and resource needs.
QUALIFICATIONS FOR IDDS RESOURCE COORDINATOR
Education and Training Requirements

• Bachelor’s Degree in Human Services or related field and 1-2 years of experience working in IDDS programs. Will consider applicants with equivalent experience.
• Competence in working with individuals who are intellectually and developmentally disabled
• Must have valid driver’s license, motor vehicle and automobile liability insurance coverage
