Nexus Family Healing is Hiring a Case Manager - Community-Based Services Near Baltimore, MD

Nexus-Woodbourne is seeking a Case Manger - Community-Based Services professional with a focus on creating and managing therapeutic treatment plans.

Position Summary: 

The Case Manager – Community-Based Services will support and reinforce the therapeutic treatment plan of clients by providing guidance and direction to residents in their daily living activities. This role will coordinate internal and external resources and activities, along with maintaining records for an assigned case load of residents.

Starting at $55,000

Primary responsibilities:

Service Coordination, Goal Setting & Progress Development

  • Collaborates with program teachers and staff, to gain a thorough understanding of the client’s case; coordinates the development of services by establishing goals and activities with assigned staff.
  • Provides one on one education and support with clients, addressing social and familial challenges; facilitates clients by re-framing problems and focusing on solutions.
  • Works with client by discussing and supporting progress towards goals and ensuring clients have a vested interest in their own long-term success.
  • Measures and summarizes client progress at specified intervals and communicates progress with staff.
  • Assists in defining relevant reporting criteria, data collection and completing project reports in a timely manner.
  • Schedules and participates in meetings with internal staff and representatives from the school, demonstrating interest in service outcomes.

Client Care & Direct Case Management

  • Educates clients on thinking errors, social and emotional awareness, parenting skills, anger management and family communication; strategically poses open ended questions helping build skills that allow clients to arrive at their own solutions long term, while providing shorter term support.
  • Actively monitors progress plans, providing continuous evaluation of plan activities and making recommendations for adjustment of services provided, for optimal outcomes.
  • Assures quality of service for clients by enforcing regulations, compliance requirements and adhering to school policy; documents all contacts including hours by type of service, and behavioral observations.
  • Provides direct service to assigned families that may include support, advocacy, role modeling, and referrals.
  • Establishes and conducts goal-oriented activities for students that aid in long term behavioral impact.
  • Completes and submits required service and case management paperwork to supervisor.
  • Remains available on a 24-hour basis by phone, for rare crisis situations that may occur.
  • Provides occasional afternoon and evening availability as needed, to attend meetings that may occur in emergency situations.

Community Work and Outreach

  • Maintains agency credibility by establishing reputable and mutually beneficial working relationships with sponsoring, advisory, and related school agencies; finds opportunities to partner and support one another.
  • Promotes the agency by ensuring an understanding of program services available for clients; publicizing activities and accomplishments and adhering to a professional code of ethics while demonstrating Trauma Informed Care.

Administrative Support

  • Participates in workshops, trainings and evidence-based programming; reviews professional publications and establishes personal linkages and application to case management work.
  • Contributes to the team by consulting and collaborating with other professionals with the shared goal of providing Trauma Informed Care with clients at the center.
  • Interact with and appreciate people from diverse, cultural, social, and religious backgrounds.
  • Continually seeks knowledge of and exhibits an understanding and acceptance of differences in gender, race, age, culture, ethnicity, class, religion and disability to better appreciate staff, children, and families.

Program Policy, Procedure & Training

  • Attends and participate in staff meetings and training sessions and required by Nexus.
  • Performs other job and work-related responsibilities and projects as needed.
  • Demonstrate problem resolution skills and the ability to handle emergency or crisis situations and use sound judgment by taking appropriate actions consistent with Nexus policy and procedures. Remain calm and effectively manage stressful workload conditions and difficult, emotionally charged or crisis situations.
  • Continually seeks knowledge of and exhibits an understanding and acceptance of differences in gender, race, age, culture, ethnicity, class, religion and disability to better appreciate staff, children, and families.

Required Education and Licensure:

  • Minimum of bachelor’s degree in Sociology, Psychology, or Social Work.
  • 2 years of experience in case management and/or Social Work.
  • Strong critical thinking, organization, and communications skills, along with attention to detail and the ability to exercise discretion with highly sensitive and confidential information.
  • Minimum 21 years of age for all clinical program serving clients older than 12 years of age.
  • Valid driver’s license required. Must meet state regulating agency and Home Office driving requirements

Preferred Education and Experience:

  • 3 years of experience in case management and/or Social Work.
