Sheppard Pratt is Hiring a Case Manager, Assertive Community Treatment Near Baltimore, MD

Assertive Community Treatment (ACT) programs provide comprehensive, mobile treatment services for adults with a persistent mental health illness. The program is for individuals who have not found success in traditional outpatient settings and who require individualized treatment services when and where it’s needed.

What to expect.

You will be responsible for coordinating and providing comprehensive direct support for your clients in accordance with the ACT model.

Additional responsibilities include:

  • Coordinating services for assigned clients, including psychiatric and medical care, housing, entitlements, etc.
  • Developing individualized service plans with clients based on their goals.
  • Providing ongoing and supportive assistance to clients.

Work hours typically follow a Monday – Friday schedule. Must have availability for occasional evening and/or weekend work. You will have independence and flexibility to schedule your appointments and complete your work under the program director’s direct approval and supervision.

What we need from you.

  • A high school diploma or GED, bachelor’s degree preferred.
  • Three years of experience working in the mental health field.
  • A driver’s license with 3-points or less and access to an insured vehicle.
