Job Summary:

The Nutrition Care Manager ensures the service, delivery, and meal assistance for Health Care (SNF, AL) residents needed to enjoy quality food by supervising and following accepted nutritional and sanitation guidelines; overseeing and setting up dining rooms, cleaning assigned areas, maintaining and restocking service areas, etc.

Essential Job Functions:

  • The Nutrition Care Manager performs clinical nutrition assessment/screening for health care residents (SNF, AL) on admission and as per policy/regulation (5-day, 14-day, 30-day, quarterly, significant change, special request, etc.) to include interview with resident/family, meal/snack observations, communication with foodservice and healthcare team, and medical record documentation.
  • The Nutrition Care Manager develops a nutrition care plan for each resident and participates with the interdisciplinary team to implement, monitor, and evaluate the care plan. Collaborates with the Registered Dietitian regarding residents at HIGH nutritional risk and monitors for high-risk parameters (weight, wounds, tube feeding, swallow disorders, eating pattern changes, etc.).
  • The Nutrition Care Manager reviews and maintains knowledge of all cycle, Always Available and other special menus, food preparation and service systems to assure compliance with facility policies, regulations and resident special requests including food service at proper temperatures, communicates with foodservice and healthcare team to address concerns/suggestions and develop ideas for quality improvement. Communicates with the Registered Dietitian regarding therapeutic diet and texture modification and medical nutrition therapy education needs.
  • Assures accurate and timely service of meals, therapeutic diets, routine, and therapeutic snacks. Assures dietary staff keep nourishment areas stocked and sanitary and that staff follows posted par sheets, schedules out of date product rotation.
  • The Nutrition Care Manager assists to monitor compliance with sanitation/safety regulations in meal service, food storage and dining service areas for health care units. Demonstrates competency of Serv Safe standards and provides education to staff as needed to assure regulatory/safety compliance. Demonstrates competency of POS system to help assure order accuracy and residents dietary requirements are being met.
  • The Nutrition Care Manager maintains written and verbal communication as requested by supervisor. Works closely with Executive Chef & Dining Services Manager.
  • Supports teamwork within the Dining Services and supports organizational mission/values as outlined in Salemtowne's Associate Handbook.
  • The Nutrition Care Manager supports Salemtowne Shines culture & philosophy.
  • Assists Dining Services with special events and other tasks as assigned at the discretion of the Director of Dining Services.

Required Skills/Abilities:

  • General knowledge of food operations, i.e., product specifications, customer service, management, food supplies, sanitation, associate supervision, etc.
  • Good communication and interpersonal skills.
  • Possess and exhibit a strong positive attitude toward customer service and have a genuine interest in geriatric care and uphold the philosophy of the community.
  • Proficient in reading, writing, and following written and verbal directions.
  • Uphold the utmost attention to the cleanliness and detail towards sanitation in food service.
  • Ability to strive to do the best job possible, to be creative and to work towards accomplishing the facility goals.
  • Capability to handle routine problems effectively.
  • Aptitude to know when to refer problems to supervisor, always has a witness present when handling discipline.
  • Possess and exhibit a strong positive attitude toward customer service and a genuine interest in geriatric care and uphold the philosophy of the community.
  • Proficient in reading, writing, and following written and verbal directions.
  • Capability of handling routine problems effectively.
  • Willingness to comply with all food safety procedures and maintain a safety-first mindset.

Education and Experience:

  • B.S. degree in Nutrition preferred.
  • Two (2) years’ experience in food service industry or Dietetics preferred.

Physical Requirements:

  • Capable of standing and walking short distances for extended periods of time.
  • Ability to lift and/or carry objects more than 50 pounds.
  • Stoop and bend at the knees, twisting from side to side at the waist.
  • Reach above, below, and at shoulder level.

Work Environment:

Work in a clean, well-lighted and ventilated kitchen and dining rooms. Possible work hazards include cuts from utensils, burns, falls due to wet floors, quick temperature changes in and out of refrigeration areas and around steam tables.
