maryleefoundation is Hiring a Case Manager Near Austin, TX

The HCS Case Manager will be responsible for managing daily operations of the HCS programs to achieve positive resident outcomes and business culture goals. The HCS Case Manager is also responsible for selecting and supervising the staff to manage a certain program or project, managing the program’s daily case manager job duties, and evaluating the success of the programs. They will oversee and implement program compliance standards as dictated by HHSC’s and other State and Federal entities. The Case Manager will enhance the quality of patient management, maximize satisfaction and promote cost effectiveness. The successful candidate will be accountable for the full patient care cycle. Case Manager duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the client’s health and human services needed.

Responsibilities:

  • Ensure that each resident receives the necessary nursing, medical and psychological services to attain and maintain the highest possible mental and physical functional status.
  • Be present for assigned shifts including carrying a cell phone when assigned and being flexible in emergency situations (On-Call).
  • Participate in professional growth activities, e.g., conferences, seminars and courses in Management Training.
  • Exhibit professional demeanor in matters of language, dress and behavior.
  • Follow and model agency policies and procedures.
  • Oversee and /or conduct all required record keeping and filing.
  • Cooperate and communicate well with Supervisors, Directors and MLF Executive Director
  • Meet on a regular basis with Assistant Director of HCS or Director of HCS
  • Use time efficiently and delegate responsibility as needed.
  • Set clear priorities and follow through on them.
  • Teach and model for staff the philosophy of independent living skills and to ensure that staff learn ways of teaching residents these skills.
  • Enable open communication with staff and to encourage staff to make recommendations regarding any improvements in the quality of residents’ lives.
  • Assist with staff training, to ensure that staff follow the daily schedule and perform assigned duties.
  • Coordinating and providing care that is safe, timely, effective, efficient, equitable, and client-centered
  • Handling case assignments, draft service plans, review case progress and determine case closure
  • Helping clients achieve wellness and autonomy
  • Responsibility to prepare for annual HHSC HCS state survey to be completed in a timely manner with the HCS team’s assistance
  • Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options
  • Develop effective working relations and cooperate with medical team throughout the entire case management process
  • Take the extra mile and interact with patients to keep track of their progress and to ensure satisfaction
  • Record cases information, complete accurately all necessary forms
  • Promote quality and cost-effective interventions and outcomes
  • Assess and address motivational and psychosocial issues
  • Adhere to professional standards as outlined by protocols, rules and regulations
  • Required to be on call one week a month.

Qualifications:

  • Bachelor’s Degree in an appropriate discipline
  • Knowledgeable in HHSC regulations
  • Must possess ability to efficiently problem-solve, plan, organize, develop, implement, and interpret programs, goals, objectives, policies, and procedures necessary for providing quality care and maintaining a sound operation
  • Must be able to maintain good staff relations and employee moral
  • Good human relation skills, strong verbal and written communication skills
  • Patience, tact, enthusiasm and positive attitude toward the elderly
  • Must be able to speak, read, write and understand English
  • Proven working experience in case management, including as a Nurse, Medical, Mental Health Care Manager or a related job
  • Excellent knowledge of case management principles, healthcare management and reimbursement
  • Previous experience with psychological aspects of care
  • Effective communication skills
  • Excellent organizational and time management skills
  • Familiarity with professional and technical emerging knowledge
  • Problem solving skills and ability to multi-task
  • Compassionate with teamwork skills

With or without reasonable accommodation, the physical and mental requirements of this job may include the following: seeing, hearing, speaking, and writing clearly. Occasional reaching with hands and arms, stooping, kneeling, crouching, crawling, frequent sitting, standing, and walking, may be required for long periods of time and may involve climbing stairs, walking up inclines, and on uneven terrain.

Additional physical requirements may include, frequent lifting, bending down, and/or moving up to 50 pounds.
