Capitol Home Health - Clinical/Field is Hiring a Rehabilitation Case Manager Supervisor Near Austin, TX

Job Description Summary

The Rehabilitation Supervisor is responsible for ensuring that patient rehabilitative care is coordinated and managed appropriately. This position is responsible for ensuring that therapy services are delivered appropriately, documentation of therapy services meets all requirements as well as the supervision of therapy professionals.

Essential Job Functions/Responsibilities

  1. Receives case referrals. Reviews available patient information related to case, including disciplines required, to determine home care needs. Assigns appropriate therapists to case.
  2. Reviews and evaluates each case by reviewing the services provided by the therapists, including case conferences and documentation review, discusses and verifies impressions, instructs and guides therapists to promote more effective performance and delivery of quality home care services, and is available at all times during operating hours to assist therapists as appropriate.
  3. Assists therapists in establishing immediate and long-term rehabilitation goals, in setting priorities, and in developing plan of care.
  4. Assists therapists in overseeing assignment and supervision of therapy assistants.
  5. Attends case conference meetings with organization personnel to facilitate coordination of care.
  6. Conducts quarterly record reviews and communicates findings and recommendations to Clinical Director and appropriate organization personnel.
  7. Completes on-site supervisory visits as necessary.
  8. Maintain accountability for therapy staff productivity, utilization and documentation standards.
  9. Assists in the screening and interviewing process of new organization personnel and makes recommendations for employment of individuals. Assists in the orientation of new therapy personnel.
  10. Assists Clinical Director in the planning, implementation and evaluation of in-service and continuing education programs.
  11. Assists in the formulation, revision, implementation, and evaluation of policies and procedures, as well as strategic goals and objectives.
  12. Complies with accepted professional standards and principles.

Position Qualifications

  1. Graduate of an accredited Physical Therapy
  2. Licensed Physical Therapist in the state of Texas with current and unencumbered license to practice in the state of employment.
  3. Two (2) years of management or supervisory experience in a rehabilitation therapy department or program, preferably in Home Health.
  4. Possess and maintain valid CPR certification. Must have obtained in-person with a practical skills evaluation as part of the final test.
  5. Compliance with accepted professional standards and practice.
  6. Has excellent observation, verbal and written communication skills.
  7. Knowledge of business management, governmental regulations and ACHC standards.
  8. Access to reliable transportation.
