COMMUNITY INTEGRATED EMPLOYMENT

JOB DESCRIPTION: PROGRAM COORDINATOR

POSITION OVERVIEW:

Mandy’s Farm provides services in the Albuquerque area for individuals with developmental disabilities, supporting them to live the life of their choosing.

The Community Integrated Employment (CIE) program supports job seekers with disabilities in identifying their strengths and interests, securing a job, and maintaining their job successfully. The primary responsibility of the Program Coordinator at Mandy’s Farm is to oversee the caseload of job seekers with disabilities and the management team (including Community Services Manager and Job Developers) responsible for monitoring the daily workings of the program. The Program Coordinator reports to the Program Director.

PRIMARY RESPONSIBILITIES

  • Understand and implement Mandy’s Farm mission, vision & values;
  • Have knowledge and understanding of all ISPs and other individual specific documentation in order to provide person centered supports;
  • Ensure program compliance with all regulatory and performance indicators as listed within the scope of service from the DD Waiver Standards and/or Grant Management Outcomes;
  • Collaborate with Program Manager, Vocational Specialist, and Program Directors to continually innovate service delivery model in line with best practices;
  • Ensure high quality service delivery through the provision of meaningful opportunities for employment, education, and skill development;
  • Demonstrate competency in all areas of Community Integrated Employment services including but not limited to: Job Development, Job Maintenance, Self-Employment, Intensive Community Integrated Employment, Community Inclusion Aide, and Group Community Integrated Employment;
  • Demonstrate competency in working with NM Division of Vocational Rehabilitation;
  • Keep abreast of best practices regarding person centered service delivery models and approaches;
  • Provide direct supervision of Program Manager; support in monitoring and managing all DSPs;
  • Provide direct supervision of Job Developers, including conducting Performance Reviews, corrective action and mediation;
  • Monitor Job Development case status and ensure funding;
  • Support Program Manager to complete Job Coach Performance Evaluations, corrective action, and mediation;
  • Attend interdisciplinary team meetings and report on persons served status;
  • Advocate with persons served to be heard in development of their outcomes;
  • Facilitate regular staffings with Job Developers & Community Service Manager;
  • Support Community Service Manager in conducting regular Job Coach staff meetings, team building activities, and training;
  • Ensure timely, accurate, and complete documentation for all assigned service recipients, including MAR, daily progress notes, event lots, timesheets, bowel logs, seizure logs, any other documentation as directed by the individual support plan and state regulations;
  • Work collaboratively with the ISP Specialist, Vocational Specialist, and Clinical Director to ensure timely and accurate reporting of outcomes and health status;
  • Participate in interagency Human Rights Committee, ensure HRC compliance;
  • Network in the local community to increase opportunities for inclusion, provide education on disability issues, and build a network;
  • Participate in routine program audits; ensure compliance and implementation of Corrective Action Plans as necessary;
  • Monitor Quality Assurance and KeyPerformance Indicators;
  • Model a professional appearance and attitude;
  • Report all concerns regarding abuse, neglect or exploitation to DHI.
  • Maintain standards of confidentiality and ethical practice; ensure program wide compliance with disclosure ethics.

CANDIDATE REQUIREMENTS

Candidate must:

  • Adopt Mandy’s Farm mission statement and values;

  • Have strong written and verbal communication skills;
  • Demonstrate exemplary multi-tasking and time management skills;
  • Be over the age of 21;
  • Have completed or in progress Associate’s or Bachelor's degree;
  • Have valid NM driver’s license and current car insurance;
  • Applicants must have a minimum 3 year of experience working with individuals with disabilities; 2 years experience of management within the disabilities service field required.
  • Completed CESP (Certified Employment Support Professional) OR commitment to complete the exam within 9 months.

Job Type: Full-time

Pay: $43,000.00 - $47,000.00 per year
