Operating out of seven locations, we offer medical care (primary care, dental, vision, OB/GYN, telehealth), behavioral health care, 340B pharmacy, nutritional assistance programs, and more.Working hours are Monday - Fridaywith occasional Saturday morning coverage.

JOB SUMMARY

The Integrated Social Worker 1 (ISW) serves as a generalist practitioner within Lower Lights Health with direct supervision. This practitioner holds both a Bachelor of Social Work degree and an active LSW license within the state of Ohio. This practitioner performs intake assessments and triage services to patients with medical and behavioral health needs within the primary care setting. Assessments include identifying presenting concerns, screening symptomology, collecting social history, and reviewing psychiatric history for diagnosis and treatment purposes. The ISW practitioner will assess, diagnose, and make appropriate referrals to clinical counseling, psychiatric services, and community mental health while providing psychoeducation to patients on their diagnosis.

DUTIES

  • Patient Care
  • Is a member of the multidisciplinary, primary care team and communicates and interacts with all levels of staff, primary care providers and patients.
  • Provides consultative behavioral health services in accordance with LLCHC expected standards at LLCHC.
  • Provides crisis intervention consultation to all members of the LLCHC provider team.
  • Performs psychosocial assessments for purpose of integrated care intervention and / or referral to ongoing behavioral health services.
  • Refers patients with severe and persistent mental health conditions to appropriate inpatient or community mental health care.
  • Selects and administers various assessment tools (PHQ-9, GAD 7) for diagnosis and treatment planning.
  • Consults with PCP/ Psychiatry concerning treatment plan and amends plan as directed.
  • Maintains charting in patient’s medical records with behavioral health diagnosis, evaluation, treatment plan and progress.

Qualifications


  • Bachelor’s degree in social work required.
  • Current license to practice as an LSW in the State of Ohio.
  • Two years of experience in the field is preferred.
  • Evidence of mature judgment, ethical conduct, and leadership skills.
  • Skill in crisis intervention and de-escalation strategies.
  • Ability to utilize assessment tools for diagnosis.
  • Ability to work with patients diagnosed with depression, anxiety, PTSD, mood disorders and other mental health disorders outlined in DSM-V.
  • Ability to work with culturally diverse patients.
  • Ability to effectively present information both orally and written to medical providers, psychiatry and other professionals connected with the physical & mental health care of the patient.
  • Ability to work positively as a team member with Behavioral Health team, Psychiatrist and Primary Care Physician.
  • Ability to work independently and implement time management strategies.
  • Computer literacy is required.
  • Must be willing to work in multiple sites as needed.
  • Awareness of personal strengths and ability to provide necessary self-care.
