Leading By Example is Hiring a Community Support Specialist Near Abingdon, MD

The Community Support Specialist (CSS) is responsible for facilitating Targeted Case Management (TCM) services for adults in the community and the Continuum of Care (COC) program in Harford County. The CSS will maintain a minimum caseload of 20 (no more than 40) TCM clients and will provide 1-5 service hours per client per month. CSS must provide a minimum of 70 billable services per month. CSS may potentially service PRP clients on an as needed basis to meet the caseload/service hour requirements.

This is a non-exempt full-time position. The salary for this position is $40,000/year. While this position can be conducted virtually with clients, on-site visits in the community in Harford County to meet clients in their homes for inspection is also required.

Responsibilities

  • Provide clients with their assessed number of TCM service hours each month

  • Facilitate individual assessments and person-centered treatment planning upon intake and on a bi-annual basis

  • Provide support and referral to community resources including, housing; links to entitlements/benefits, and behavioral and somatic health care providers; assist in developing social support systems and monitoring of engagement with services

  • Implement interventions identified in behavior and/or rehabilitation plans

  • Facilitate family support sessions with the client

  • Provide emergency crisis intervention and conflict management to identified clients and families

  • Ensure clients receive assistance and support with physical and somatic health management needs which include understanding and managing medication

The CSS will also be responsible for completing all documentation requirements and participating in coordination between care teams.

Qualifications

  • Bachelor?s degree in a human services or related field required

  • Minimum of one year proven experience in the mental health field

  • Possess a valid Maryland Driver?s License and current automobile insurance

  • Superior communication skills

  • Skilled networker

Preferred Qualifications

  • Three to five years of experience in human services or related field

  • Proficiency in Spanish
