1.Initiates denial and appeal process in accordance with policy for current Rehab patients
2. Identifies and intervenes to coordinate care on patients in relation to appropriateness of level of care length of stay and resource utilization issues
3. Provides initial and ongoing psychosocial assessments for patients/families having complex psychosocial needs and/or medical complex diagnosis. Ensures patient is progressing towards desired outcomes and identifies barriers that hinder effective patient care.
4. Documents comprehensive social work assessments, treatment goals, interventions, progression and discharge plans in the patient’s medical record congruent with departmental policies.
5. Demonstrates leadership in the professional development of self and others
6. Communicates and works effectively with patients, family members/significant others, and other healthcare professionals.
7. Participation in the interdisciplinary treatment planning process, attending team meetings, and working in conjunction with the treatment team.
8. Performs concurrent review for all payers and maintains documentation as directed by the Rehab Department.
9. Identifies, tracks, and records Avoidable Care Days. Intervenes to prevent or decrease ACD as appropriate.
10. Provide weekly updates, education, and encouragement to patient/family
11. Provides crisis and supportive counseling to enhance the patient’s and family’s problem solving and coping skills .
12. Identifies and resolves barriers that hinder effective patient care
13. Facilitate collaboration with patients existing sources of support; identify new sources of support
14. Educates patients and families regarding post discharge needs and provide resource information
15. Coordinate discharge planning activities
16. Initiate community/facility referrals based on identified patient/family needs.
17. Collaborates with community agencies to provide services to assist the needs of patients/families post discharge.
18. Contribute to quality improvement activities through self-analysis of services and needs.
19. Initiate education activities with and for facility staff related to ethical, psychosocial and discharge planning issues.
20. Maintains current knowledge of trends related to medical social work health care issues through attendance and active participation in continuing education and professional organizations.
21. Facilitates post discharge follow-up calls to ensure patient/family’s needs have been met
22. Assist PPS Coordinator with Team Conference documentation
23. This position will float as needed based on IPR census
24. This position may supervise social work students on a voluntary basis
25. Assumes additional responsibilities
QUALIFICATIONS:
EDUCATION
Required: Master’s Degree in Social Work or Human Service Field
Preferred: Current Licensure by the Commonwealth of Virginia as a Licensed Clinical Social Worker
EXPERIENCE
Required: Experience in discharge planning or case management
Preferred: Two years’ experience in a hospital or health care setting
