  Position Summary

  Serves as a member of interdisciplinary Hospice Team, assessing the social, emotional, psychological needs of the hospice patient and family. Plans appropriate intervention strategies and care plans to facilitate maximal functioning of both patient and family in the patient's home setting (private home, long-term care facility, group home, etc.).

  Provides grief/loss counseling, community resource referral, social service coordination and other services relating to the patient's terminal condition.

  Job Qualifications

  • Work requires knowledge of casework and public welfare principles at a level generally acquired through completion of an accredited Masters Degree program in Social Work, with one to two years progressive experience preferred.
  • Work requires knowledge of federal, state, and local regulations related to hospice; knowledge of health and welfare resources and facilities; knowledge of insurance benefits, including Medicare and Medicaid reimbursement policies; knowledge of medical, legal and financial implications for patients and their families, particularly those relating to end-of-life decisions (advanced directives, medical power of attorney, Do Not Resuscitate orders, guardianship, etc.) and age-specific service needs.
  • Work requires previous experience working with and addressing the needs of dying patients and their families; experience working collaboratively in a team-based setting; and experience in patient assessment, care planning, resource referral and counseling service provision.
  • Must have valid driver's license and an acceptable driving record.
