**Job Overview:**

We are seeking two dedicated Case Managers to join our team. The HCS Case Manager works collaboratively with Service Coordinators from Local Authorities to ensure high-quality services are provided to individuals. This role ensures that individuals' needs are met according to their preferences and goals, adhering to all federal and state regulations as outlined in the Home and Community-Based Services (HCS) Texas Administrative Codes (TACs). The HCS Case Manager may also train direct care team members, provide on-call coverage, and requires local travel.

**Duties:**

• Person Directed Plan (PDP) Participation:
• Engage in the PDP process with the Local Authority.
• Generate and maintain documentation for Individual Plans of Care (IPCs), Intellectual Disability/Related Condition (ID/RC) forms, Implementation Plans, and other necessary documents.
• Service Coordination:
• Initiate and maintain services as described in the PDP.
• Coordinate services provided by team members and contractors.
• Monitor progress and communicate updates to the Interdisciplinary Team (IDT).
• Documentation and Communication:
• Coordinate and document IDT meetings.
• Ensure timely completion and sharing of required documentation with individuals, their Legally Authorized Representatives (LARs), Service Coordinators, and other team members.
• Update documentation following changes in individuals’ lives in conjunction with the Service Coordinator.
• Goal and Outcome Monitoring:
• Review progress towards goals and outcomes with individuals and their families or LARs.
• Revise plans and programs as needed to meet individuals’ needs and goals.
• Ensure timely notifications and required documentation for emergency services, restraints, hospitalizations, DFPS allegations, or other unplanned events.
• Individual Rights and Money Management:
• Protect and promote individuals' rights to manage their own money.
• Oversee documentation regarding room and board, Trust Fund Expenditures, and spending money upon request.
• Case Management Services:
• Ensure delivery of case management services, as evidenced by individual and family satisfaction.
• Monitor caseloads, evaluate treatments and responses, advocate for needed services, obtain additional resources, and intervene in crises.
• Review and adjust consumer utilization sheets for over/under utilization.
• Training and On-Call Duties:
• Train direct care team members as needed.
• Provide on-call coverage for the agency.
• Other Duties:
• Perform all other job duties as assigned by the Supervisor.

Working Conditions:

• This position requires local driving and on-call responsibilities.
• Work is typically performed in both office and community settings.

Physical Requirements:

• Ability to perform job duties that may require standing, sitting, and walking.
