ABOUT IMPACT HEALTH
Impact Health is an Asheville-based nonprofit leading Western North Carolina’s participation in North Carolina’s Healthy Opportunities Pilot. HOP is a groundbreaking initiative that uses Medicaid funding to support social drivers of health across four key domains: food, housing, transportation, and interpersonal safety and toxic stress. The pilot’s aim is to improve health outcomes for NC Medicaid members who could benefit most while reducing overall healthcare costs.
Impact Health supports a network of more than 50 human service organizations (HSOs) by:Serving as a bridge between the state, private health plans, care managers and other HSOs.
  • Building organizational capacity through financial support, training, and technical assistance.
  • Assisting with Medicaid billing, data collection and evaluation efforts.
  • Convening pilot entities to share best practices.

SUMMARY
The Community Engagement Manager (CEM) serves as the primary point of contact within their region for HSOs, supporting the HSO network to ensure each HSO can provide high quality services to HOP participants and meet the various requirements of the Pilot program.
This position supports HSOs with receiving technical assistance, building awareness, and acting as a liaison and resource person for all Pilot program stakeholders working within the catchment area. CEMs are "utility players" who use their deep knowledge of community-based organizations and resources to accomplish a wide range of tasks. While all CEMs will participate in all the following functions, some may specialize more in certain areas.
The region this CEM role will serve, and be actively present in, ideally would be the region they live in. We are currently seeking someone that either lives in our western or eastern most boundaries. Our western-most counties of Cherokee, Graham, Clay, Macon, Swain, Jackson; our eastern-most counties include Burke, Avery, Mitchell, Yancey, McDowell, Rutherford, and Polk.
ROLE
HSO Network Management Support:
  • Serve as the primary liaison for network HSOs by building strong relationships with HSOs, supporting them to troubleshoot issues and maximize their Pilot program participation.
  • Conduct ongoing monitoring of HSO network capacity as it relates to the demand for Pilot program services in WNC.
  • Assist in ensuring that the network capacity is sufficient to meet the Pilot program demand, working to ensure Pilot program services are available to meet the needs of eligible Medicaid enrollees who are referred by care managers.
  • Inform the Director of Programs of relevant concerns of day-to-day operations as it relates to the HSO network.
  • Work to support HSOs with developing their processes and capacity development, including data entry and tracking processes.
  • Provide technical assistance with the referral process, and on technology and general business practices to ensure an efficient service delivery process.
  • Attend and present at stakeholder meetings as requested.
Training and Technical Assistance (TA) Support:
  • Conduct educational webinars and live training events for human services organizations (HSOs) throughout the WNC region.
  • Provide ongoing education to HSOs on the expectations for Pilot Program Implementation, conducting in person group convenings and regular telephonic or web based convenings. Training topics will be based on HSO capacity assessments.
  • In collaboration with other staff members (e.g., billing, compliance), develop and disseminate monthly informational bulletins related to training topics.
  • Support the development and implementation of the Network Lead Training and TA Plan, which includes training for HSOs and other Pilot-Participating Entities (e.g., the Care Managers who will be making referrals) to support Pilot Program success.
  • Support HSOs to build capacity related to NCCARE360, including data collection, management, and reporting, through targeted TA and information shared in bulletins.
  • Leverage regional champions and implement a train-the-trainer approach to support the engagement of new entities and the spread of best practices.
  • Engage in practice scenarios to develop critical workflows with our network HSOs.
  • Coordinate and participate in a variety of convenings with pilot-participants and nonparticipants, including regional and network-wide convenings.
  • On an ongoing basis, maintain documentation of input received from all stakeholders related to training activities, using input received to inform training and TA activities.
General Support:
  • Assisting care managers by providing technical assistance and working as a bridge for HSOs working with HOP clients.
  • Assisting potential clients with entry into HOP pre-screening through NCCARE360.
  • Assisting HSOs with invoicing questions and when needed, approving invoices within NCCARE360.
REQUIREMENTS

Education & Experience
  • Associate degree in public health, social work, or related field required with four years’ experience in the areas of social services, network administration, and/or utilization management.
  • Bachelor's degree in public health, social work, or related field with two years’ experience in the areas of social services, network administration, and/or utilization management. Knowledge of needs and issues related to social determinants of health, preferably with a focus in housing and/or transportation.

Working Conditions
Position is remote with occasional to frequent visits to external environments that require driving to and from various locations including the office as needed.
